The ‘Living Well with Chronic Conditions’ workshop offered by the Minnesota Dept of Health and CommonBond Communities provides a series of workshops for patients, caregivers, family and friends to learn how to reduce their symptoms, increase health and improve life.
It is an ‘evidence based program’ which has proven results that was developed by Stanford University’s Patient Education Research Center over 20 years ago and adopted by groups including Kaiser Permanente.
The classes help people learn to use problem solving and action planning strategies to live well with chronic conditions. The leaders are effective in helping participants develop new techniques that can be applied to everyday life.

Participants learn ways to deal with pain, fatigue and the dietary or other activity changes they need to make to improve their health. Dealing with the emotions that accompany chronic conditions such as: frustration and depression is also addressed. Participants learn how to communicate their needs and frustrations to doctors and how to help friends to understand.
The workshops offer a wealth of wisdom including tips to help manage stress, as well as, insight on recognizing symptoms that may need professional attention. People offer each other ideas on coping and pair up as buddies to encourage each other to meet short-term goals.
