MnCHOICES Assessment Team Offers Help For Those In Need Of In-Home Assistance

Are you concerned about the welfare of a loved who needs extra help with taking care of themselves at home? Do you know someone that needs senior living care? Services to help with dressing, bathing, meal prep, medication management and/or assistance with maintaining health and safety are all available. A MnCHOICES assessment could be the place to start. Family and/or any other concerned person (friends or neighbors) can call the MnCHOICES Assessment Team to make a referral. We are here to strengthen, serve and support each member of our community.
