Carlyle Place, A Courville Community Receives Deficiency Free Survey

Carlyle Place, a 46-bed assisted living center located in Bedford, is proud to announce they have received a deficiency-free survey from the State of New Hampshire Department of Health & Human Services. The center, which is one of four Courville Communities in southern New Hampshire, received the deficiency-free survey following an impromptu visit by a licensed surveyor from the state of New Hampshire.

Throughout the survey, reviews were performed of resident charts where they search for proper documentation, signed physician orders for all medications (including OTC), proper completion of Resident Assessment Tools and all corresponding paperwork including nursing assessments and care plans.  A tour of the facility was conducted to ensure the building is properly maintained, all inspections have been completed in accordance with HFA rules (fire / sprinkler systems, employee training on emergency preparedness), and to inspect the cleanliness of the building. Surveyors met with residents to see if they have any comments, concerns or complaints that need to be addressed.

“A deficiency-free survey doesn’t happen because you’re doing well the day that state surveyor walks in the door” says Alyssa Rifkin, Executive Director of Carlyle Place. “A deficiency-free survey is the result of a dedicated and hard-working team making our residents’ health and safety their top priority 24 hours a day, seven days a week.”

New Hampshire assisted living centers are licensed and governed by the state. Centers can carry one of two licenses, and 804 or an 805 license. Carlyle Place carries an 805 license, designating them as a Supported Residential Care Home.
