Rank is determined by the percentage of hospitals in a state that received an "A" rating from Leapfrog. In New Hampshire Island, four of the state's 12 hospitals received "A" ratings in Leapfrog's Fall 2016 assessment, while four other hospitals received a "B" and the other four a "C".

The four which recieved an "A" rating were Concord Hospital, Frisbie Memorial (Rochester), Chesire Medical (Keene), and Portsmouth Regional.

The safety grade is based on factors such as how often patients at a given hospital acquire infections or other conditions during their stay, or how well-staffed a hospital's intensive care unit may be.
