Open Registration for Clark Pop Warner 2011 Season

Open Registration for the fall 2011 football/cheering season for all football players/cheerleaders will take place on three nights, April 5, 6 and 7 at the Pop Warner Clubhouse from 6 to 8 p.m. 

Payment must be made at time of registration as follows:

1 child:  $100 + $100 for raffle ticket booklets

2 children:  $150  + $150 for raffle ticket booklets

3 children;  $200 + $150 for for rafle ticket booklets

You must also bring 1 copy of your medical insurance card, 1 copy of child's birth certificate, 1 photo of child.  Registration will not be accepted without required forms.

Prior to first practice you must also provide 2 copies of child's report card and completed Doctor's Medical Form.
