Monmouth Medical Center, Southern Campus Presents Five Wishes

In order to raise awareness for advanced care planning, Monmouth Medical Center, Southern Campus will present Five Wishes

According to the Center for Disease Control and Prevention (CDC) 70 percent of Americans do not have an advanced care plan. There are many things in life that are out of our hands, but our wishes about advanced healthcare treatment are not. In order to raise awareness for advanced care planning, Monmouth Medical Center, Southern Campus will present Five Wishes, on Friday, October 7 from 2 to 3 p.m. at the Ocean County Library’s Manchester Branch, located at 21 Colonial Drive in Manchester.

Led by Claire Verruni, RN, an expert in palliative and hospice care at Monmouth Medical Center, Southern Campus, attendees will learn about the importance of advanced care planning and the Five Wishes document. An easy to complete form, the document allows individuals to specify exactly what kind of health measures they want taken down the road and can help family members and loved ones make important decisions. Once the form is filled out and properly signed, it is valid under the laws of most states.
