Monmouth County Sheriff's Office Ident-Adult program.
Ident-Adult are photo ID cards that are provided free to all Monmouth County residents age 50 and older. They can be life-saving during emergencies, contain information such as medical conditions, allergic reactions, doctor information and phone numbers, and can be used as a convenient secondary form of identification.
A registration form must be completed before going to the fire house. You must bring the form and proof of identification to the fire house to be photographed. Proof id identifaction can be one of the following: driver's license, Medicare card, Medicaid card, Social Security card or voter registration card.
