The Randolph Township Health Department has received grant funding to assess the social and health effects of COVID-19 on the municipalities served.

A brief survey has been developed to identify impacts and needs of community members. The goal is to ensure that at-risk residents in our communities have access to testing, vaccination and support services.
The survey may be accessed here. Surveys must be completed by Friday, July 30, 2021.

All information provided in the survey is anonymous and kept confidential; it is important for participants to be as accurate as possible.
Hard copies of the survey, along with resources in Spanish and English, are also available at the municipal building and the library.
