What is Senior Reach?
Senior Reach provides free support for the well-being, independence and dignity of older adults through community education, mental health services, and connection to community resources.
· It’s a community program that identifies older adults who may need emotional or physical support and/or connection to community services.

How Does Senior Reach Work?
The Senior Reach relies on community members - friends, family, neighbors - to help identify older adults who may need help. Community members can contact our call center to discuss their concerns and make a referral. A Senior Reach Specialist will talk with the senior to determine need and willingness for intervention. If the older adult agrees, Senior Reach will help connect them with appropriate services.
Senior Reach Services
· Free in-home counseling
· Referrals to transportation services, home health aids, Medicare cost saving programs, and more
· Connection to community resources
· Depression screening and treatment
