A Patient Practice Portal and electronic records was recently implemented at Old Hook Medical Associates in Emerson, according to a news release.
The change was "to help make a better connection between patients and their healthcare providers," the release said.
New patients can fill out pre-visit questionnaires, registration forms and medical insurance information to create a patient chart before their first visit, the release said. All further updates, treatments and follow ups will be updated online.

“We’ve embraced this technology and believe that it will benefit our patients greatly," Edward Gold, from Old Hook Medical Associates, said in a release. “We’re a multi-specialty practice so the ability for any of the physicians treating a patient to seethe full medical record is essential. Electronic Medical Records gives us a 360 degree view of the patient and the Practice Portal ensures that the information is available to the patient 24 hours a day.”
