Albuquerque Public Schools: COVID Surveillance Testing For APS Employees

Albuquerque Public Schools employees are required to participate in weekly COVID-19 surveillance testing if one of the following applies:

- Employee isn't fully vaccinated against COVID-19
- Employee opted not to disclose COVID-19 vaccination status
- Employee is fully vaccinated against COVID-19 but hasn't completed the exemption survey
Employees required to test weekly shall do so on their own time (i.e., outside of the duty day or while on leave during the duty day).
- Select one of the three COVID-19 testing options listed below.
- Take your test during the assigned testing window; the sooner, the better.
- Send your test result to your supervisor as soon as possible but no later than the Monday following your assigned testing window.
Testing Option 1: Premiere Medical Group On-Site Test

- Location: Berna Facio Professional Development Complex, 3315 Louisiana Blvd NE
- Times: 7:15 a.m.-3:30 p.m. Monday through Friday
- Dates: Oct. 18-Dec. 21. Closed Nov. 15, 25 & 26
- Register online External link (one-time registration). If you cannot register online, registration forms will be available on-site (please allow for more time.)
- Fill out an online intake form.
- Be prepared to show your QR code at the time of the test.
Testing Option 2: Vault Health At-Home Test
- This test is self-administered and can be taken at home or work.
- Select Vault Health as your testing option External link opens a new window.
- Order an at-home, saliva-based COVID-19 test.

Testing Option 3: Department of Health Curative Testing Site
- Select a Curative testing site from the list provided on the Department of Health website: COVID-19 Screening & Testing | NMDOH - Coronavirus Updates External link opens a new window. .
- Refer to the public health screening and COVID-19 testing sites.
If you are fully vaccinated, you are eligible to become exempt from required weekly COVID0-19 surveillance testing.
- Complete the COVID-19 Testing Exemption Surve External link opens a new window. y - be prepared with your COVID-19 vaccination information and ensure you're logged into your APS Google and Gmail account.
- Once you complete the exemption survey, you will be removed from the surveillance testing list 14 days after receiving your second Pfizer or Moderna vaccine dose or the single Johnson & Johnson vaccine dose. Until then, you will be required to participate in surveillance testing.

- If you have already completed the survey but remain on the surveillance testing list, return to the survey and verify the date listed for your final vaccine dose is correct.
