  • Monday, Aug. 2, from 2-6 p.m. at La Cueva High School, 7801 Wilshire Ave. NE
  • Wednesday, Aug. 4, from 2-6 p.m. at Cibola High School, 1510 Ellison Dr. NW
  • Thursday, Aug. 5, from 2-6 p.m. at West Mesa High School, 6701 Fortuna Rd. NW 
  • 4 or 5 doses of Diphtheria-Tetanus-Pertussis (DTaP)
  • 3 doses of Hepatitis B (Hep B)
  • 3 or 4 doses of Polio (IPV)
  • 2 doses of Measles-Mumps-Rubella (MMR)
  • 2 doses of Varicella
  • 2 doses of Hepatitis A (Hep A)
  • 1 dose of Tetanus-Diphtheria-Pertussis (Tdap)
  • 1 dose of Meningococcal (Men ACWY) (required 7th-9th grade; recommended 10th-12th)
  • 2-3 doses of Human Papillomavirus (HPV) (Not required; highly recommended)
