Dr. Khan has served as an elected board member of the New York State Academy of Family Physicians from 2023 to 2026 and was Chair of the organization's Diversity, Equity, and Inclusion Commission during that time.

She earned her Doctor of Medicine degree from Ross University School of Medicine in 2001 and completed her Family Medicine residency at UChicago Medicine LaGrange Hospital in 2004. She is board certified by the American Board of Family Physicians.

During her career, Dr. Khan has been affiliated with Montefiore Medical Center and has served on the faculty at Albert Einstein College of Medicine from 2006-2016.

Dr. Khan says, "As an independent, community-based organization, Long Island Select Healthcare offers a unique opportunity to make a meaningful impact. Its patient-centered mission and the dedication of its staff are truly inspiring, and I look forward to helping advance its continued growth and reach. My goal is to ensure that LISH Care continues to lead as a health care organization asset—especially for individuals with intellectual and developmental disabilities and others with complex needs. By strengthening care coordination, expanding community partnerships, and focusing on whole-person, culturally responsive care, we can improve access and outcomes for the patients who rely on us most. I see tremendous opportunity to build on LISH Care's strong foundation and further engage its potential across Long Island."
Long Island Select Healthcare is observing its 10th anniversary. It is a prime example for the continued progress NYS OPWDD has been making in this field. In addition to its Central Islip headquarters, LISH Care has clinical sites in: Hauppauge, Smithtown, Center Moriches, Port Jefferson Station, Manorville, and Riverhead.

LISH Care is a 16-specialty Article 28 Federally Qualified Health Center and a NYS Article 16 Rehabilitation Clinic. About 65% of the LISH Care patient profile is people with developmental disabilities, autism, and special needs.
Financing of LISH Care is based on the patient's family income and size, with a mix of funds from both Medicaid and Medicare. Fundraising efforts are encouraged by the Federal government, and LISH Care has a program for donations and contributions underway.
LISH Care currently provides a variety of services to almost 9000 enrolled patients. In 2025, it recorded 74,881 visits. LISH Care is an employer of about 160 people.
