United Hands Organization is a part of New York State’s 1115 Waiver Medicaid Social Care Network (SCN), which connects Medicaid members to essential social care services that support health and well-being. Our team helps individuals identify unmet social needs and connects them to resources such as food assistance, housing support, transportation, and other community-based services. We also provide ASL-directed services to ensure accessibility for Deaf and hard-of-hearing community members. We aim to make it easier for people to access the support they are eligible for and improve their overall quality of life. We welcome community members to learn more about available services, ask questions, and explore how we can help connect them to the resources they need.
