A representative from the Occupational Safety and Health Administration — the federal agency that regulates worker safety — told the Daily News the agency had opened an investigation into the fall on Monday.

"The scaffold holding the worker (fatality victim) went vertical, causing the worker to fall from the scaffold," the spokeswoman told the Daily News.
