The winning ticket, worth $59,254.50, was sold at Bar Jay Cards on 3-10 North West Drive.

In addition to the $59,254.50 top prize for the September 12 TAKE 5 drawing, 169 players each purchased winning tickets worth $525.50 by matching four out of five numbers for the drawing.
In total, there were 78,187 TAKE 5 winners for Monday's drawing.

The winning numbers for the September 12 TAKE 5 drawing were 03-12-22-35-37.
The winners may claim the tickets at any one of the Lottery’s Customer Service Centers statewide, including the Long Island office at 45 South Service Road, Plainview, NY 11803 or at Resorts World Casino New York City, 110-00 Rockaway Blvd., Jamaica, NY 11420.
