SAFE Coalition: On Opioid Treatment Program Healthy 2020
The goal of Nassau County Opioid Treatment Program Healthy 2020 is to address substance use and physical health.

The Nassau County Department of Human Services May Newsletter discussed opioid treatment and their proactive plans to facilitate health and wellness. The goal of Nassau County Opioid Treatment Program Healthy 2020 initiative is to address substance use and physical health concurrently, and to assist patients in achieving optimal health outcomes.
Nassau County Opioid Treatment Program’s Healthy 2020 initiative will utilize existing staff to facilitate access to health education materials, along with care and skills training related to existing medical conditions and or risk factors for other chronic health conditions. Patients identified by County medical or counseling staff as having a need for additional health intervention, will be referred to the Healthy Patient Education program for a one on one educational intervention. The patient educational initiative will be provided by the program’s designated nursing staff. A designated health office will be utilized to meet with patients who have been identified as having an identified need.
These efforts are aimed at reducing hospitalizations and the barriers to health care that patients often face and to also motivate the patient to become actively involved in their health care maintenance. The impact of the initiative will be monitored by tracking of hospitalizations and patient adherence.
