Suffolk County Executive Steve Bellone announced that a new COVID-19 vaccination clinic for children ages 5-17 will open on November 11. The clinic is located on the south-facing side of the H. Lee Dennison Building at 100 Veterans Memorial Highway in Hauppauge.

Appointments are not required, however, those with appointments will have priority. Visit https://vax4.suffolkcountyny.gov/ or call 311 to schedule an appointment.

For children ages 5-17 of age, a parent or legal guardian must provide consent for vaccination, either in person, by phone at the time of the vaccination appointment, or by completing a consent form. Minors who are 5-15 years of age must be accompanied by an adult caregiver.
