2025 SHPBest Patient Experience Award Goes To Montefiore Nyack Hospital Home Care

NYACK, NY — Montefiore Nyack Hospital Home Care has been recognized by Strategic Healthcare Programs as a 2025 SHPBest “Superior Performer” for patient experience.

The program earned an overall patient experience score that ranked in the top 20 percent of all eligible SHP clients for the 2025 calendar year.

The annual SHPBest award program was created to recognize home health agencies that consistently provide high-quality service to patients. The 2025 recipients were chosen by reviewing and ranking the overall experience score for more than 3,200 home health providers.

SHP uses what it described as the nation’s largest Home Health Care Consumer Assessment of Healthcare Providers and Systems (HHCAHPS) benchmark to identify organizations that made patient experience a priority and received high marks on the HHCAHPS survey.

"Providing excellent patient care takes relentless effort, and our SHPBest award winners embody that every year," said Jason Ballet, President of SHP. "This program lets us recognize the organizations that never settle, that keep improving the patient and caregiver experience even when they're already performing at a high level. That kind of commitment deserves to be celebrated."

This is the fourth consecutive year Montefiore Nyack’s Home Care program has been recognized as an SHP Superior Performer. The program also was named to U.S. News & World Report’s inaugural Best Home Health Agency list in 2026.

“The patient experience is at the heart of our Home Care Program,” said Colleen Nunes, home care administrator at Montefiore Nyack Hospital. “This recognition reflects the dedication of our entire team to providing compassionate, dignified, and exceptional care to the patients and families we serve in our community.”

Montefiore Nyack Hospital is a 375-bed community acute care medical and surgical hospital founded in 1895, according to the announcement. The hospital said its mission is to provide emergency and acute care services to Rockland County residents and surrounding areas. It is a member of Montefiore Health System and partners with Albert Einstein College of Medicine and Touro College of Osteopathic Medicine on clinical rotations for medical students.
