PATCHOGUE, NY — The Patchogue Theatre for Performing Arts will require guests attending events provide valid proof of vaccination for patrons 12 and over or a negative COVID-19 test before entering the premises starting on Oct. 1, according to an email to patrons.

Acceptable forms of proof of vaccination include the New York State Excelsior Pass and a Centers for Disease Control and Prevention vaccination card, or a photograph of it.

COVID-19 test results must include a printed or digital negative COVID-19 PCR or rapid antigen test administered by a healthcare professional within three days before the event and include the person's name and the date of the test.

It must include the child's name, the date of the test, and the child must be accompanied by a vaccinated adult.

Home COVID-19 testing results will not be accepted as proof of negative test status.

All proof must be accompanied with a valid identification matching the name on the person's documentation.

The theatre is also requiring masks for all patrons — both vaccinated and unvaccinated — to be worn during events.

The email then goes on to include a link where patrons can find COVID-19 testing through the U.S Department of Health and Human Services.

"The safety of our guests, staff, and working crew are of the utmost importance. We need your help and cooperation to provide everyone with a safe and enjoyable experience," the email read.

Theatre officials say they will consider the return or exchange of tickets for patrons who cannot attend due to their vaccination or testing status.
