Alzheimer’s and Dementia Care Consultation for Caregivers

Alzheimer's and Dementia

If you are helping care for a family member or friend with Alzheimer’s disease or other forms of dementia, you are not alone! We can help.

Our social workers are available to meet with family caregivers and their care partners to serve as care navigators by providing information about how to speak with your doctor about memory or other problems, available resources, use of support services; and providing emotional support. Our social workers work in partnership with caregivers, the person with dementia and other family members to assist you in planning for the future, linking you to community resources, giving you information about your loved ones condition, and providing referrals to other services that you may need.

Consultations can be done anywhere that is convenient for you; in the home, by telephone, or at another location in the community.

Our staff is also available to meet with you and other family members as a group to discuss areas of concern, and work collaboratively to develop a plan to help the person living dementia.
