Obituary: ​Josephine “Jo” Zalak, of Smithtown, Dies at 68

Died: August 28, 2016

Visitation: Wednesday, August 31 from 5 p.m. to 9 p.m. and Thursday, September 1 from 9 a.m. to 10 a.m. at the Five Smithtown Funeral Home on 31 Landing Avenue in Smithtown.

Service: Thursday, September 1 starting at 10 a.m. at the Five Smithtown Funeral Home.

Obituary: Josephine “Jo” Zalak, (nee Forcella), of Smithtown, passed away on Sunday, August 28. She was 68.

Zalak was born to mother Edith Forcella. She retired from Smithtown Personnel Department and a former member of Smithtown Ladies Auxiliary.

She was married for 40 years to Edward Zalak.

Zalak is survived by daughters Maria Francati, Jennifer (McCoy) and Laura (Soos). She is also survived by her grandchildren Hailey, Juliana and Jillian as well as her sister Janice Khalsa.
