It's Your Move to Better Health: Type 2 Diabetes
Diabetes Self Management Program at Putnam Independent Living Services
The Diabetes Self Management Program, a free six week workshop to reduce the impact of Type 2 Diabetes among individuals with a disability is now available through Putnam Independent Living Services.
Diabetes affects 25.8 million people and is a leading cause of death in the U.S. population. Left uncontrolled diabetes is a cause of kidney failure, non-traumatic lower limb amputation, new cases of blindness, and is a major cause of heart disease, stroke and hospitalizations.
For individuals living with diabetes managing the symptoms and maintaining an active, fulfilling life can be challenging. Workshop sessions address key topics including monitoring your blood sugar, a healthy eating plan, preventing or delaying complications, physical activity, dealing with stress and difficult emotions, working with your health care professional and making an action plan.

Individuals with any type of disability, including those age related, are invited to participate. Workshops will be held at both Putnam Independent Living Services (Carmel, NY) and Westchester Independent Living Center (White Plains, NY). About Putnam Independent Living Services and Westchester Independent Living Center
Putnam Independent Living Services is a satellite office of Westchester Independent Living Center, a nonprofit, community-based; consumer directed organization dedicated to improving the lives of people with disabilities and has a 30 year history of providing information, services, advocacy, technical assistance, and trainings in the Lower Hudson Valley Region to diverse and/or underserved populations. Our mission is two-pronged: to assist individuals with disabilities become as independent as they can be and to work within the community to provide education, disability awareness, and advocacy for the removal of barriers. The programs and services offered are designed by, directed by, and, in most cases, delivered by individuals with a wide range of disabilities. Although the bulk of the services are based in Westchester and Putnam, we also serve Dutchess, Orange, Sullivan, Rockland and Ulster Counties. Core services include benefits advisement, information and referral, community outreach.
