Pulmonary Partnership between White Plains Hospital & Cabrini of Westchester Recognized as a Best Practice by PointClickCare

PointClickCare Is A Leading National Electronic Health Record Organization

A partnership between White Plains Hospital and Cabrini of Westchester was cited as a best practice in transitions of care for pulmonary patients at the recent PointClickCare SUMMIT, a national conference for healthcare professionals in Orlando, Florida.

Recognizing that individuals with chronic lung disease represent a significant number of re-hospitalizations both nationally and locally, Cabrini of Westchester established a Pulmonary Rehabilitation program to serve as a step-down program for patients being discharged from area hospitals with a primary or secondary diagnosis of chronic lung disease. In an effort to reduce re-hospitalizations and offer pulmonary patients optimal outcomes, White Plains Hospital partnered with Cabrini of Westchester and developed protocols to transition patients to Cabrini of Westchester’s Pulmonary Rehabilitation Program and/or Certified Home Health Care Program. As a result of the partnership, Cabrini of Westchester’s Pulmonary Program has enabled White Plains Hospital to decrease its pulmonary-related readmissions from 24% to 16.8% in the first year and increase quality outcomes and patient satisfaction.

Lorraine Horgan, MS, Vice President of External Affairs of Cabrini at Westchester and Karen Hanney, MS, FNP-BC, CMAC, Case Management/Utilization Administrator at White Plains Hospital presented this data from their pulmonary partnership at the PointClickCare Annual SUMMIT in Orlando, Florida earlier this month. “An estimated 250 attendees at the presentation were clearly enthused to learn about how they could adopt this best practice in their healthcare communities,” said Hanney. ”We are gratified that by sharing the Cabrini/White Plains Hospital experience, we were able to provide the audience with a deeper knowledge of the latest available tools, tips and best practices to enable them to deliver quality care to the growing, medically complex senior population,” she added.
