North Coast Health Is Now Neighborhood Family Practice
Beginning January 2, you may continue going to the Lakewood location (now renamed Neighborhood Family Practice Health Center) for care.

From NFP:We value you as a patient and are pleased to announce that effective January 1, 2019, North Coast Health will begin operating as Neighborhood Family Practice (NFP). You will continue to receive the same quality care and service with NFP that you have come to know with North Coast Health, as NFP shares the same dedication to delivering high quality affordable care to patients and the community.
To make the transition as easy as possible for you, the information below highlights some things you need to know about our services going forward:
- The North Coast Community Health Center will be closed from Wed., Dec. 26, 2018 through Tues., Jan. 2, 2019 and will reopen Jan. 2, 2019 as Neighborhood Family Practice. If you need medical care during this time, you may call 216.228.7878 for assistance scheduling an appointment at one of NFP’s five nearby locations.
- Beginning January 2, you may continue going to the Lakewood location (which will be renamed Neighborhood Family Practice North Coast Community Health Center) for care. You will also be able to go to any of our five other community health centers which offer additional services including pregnancy care, psychiatry and dental.
Financial Assistance/Charitable Care:
Similar to North Coast Health, NFP offers a sliding fee discount based on family size and income that has been expanded to include patients with income at or below 400% of the federal poverty level.
Effective Jan. 1, 2019, the North Coast Health Charitable Care program will end. If you are currently participating in this program, you will need to enroll in NFP’s financial assistance program prior to making your next appointment. The cost of visits ranges from $5 to $45 depending on income and family size.

- If you have already scheduled an appointment at North Coast Health in 2019, you will be able to keep your appointment. You will be contacted to arrange a meeting with an NFP patient benefits specialist who will answer questions about health care benefits, assess your health insurance options, provide information on basic health insurance issues and assist with applying for coverage and accessing benefits.
- If you have questions about NFP’s financial assistance program or if you would like to make an appointment with a patient benefits specialist, please call 216.228.7878.
Pharmacy:
- NFP participates in the 340B Drug Pricing Program that allows us to purchase medications at a discounted price. As a result, we can make these medications available to our patients at a lower cost than most other pharmacies.
- You may have received free sample medications from North Coast Health. NFP does not provide free medication samples, but we will work with you through the 340B Drug Pricing Program and pharmaceutical company discount programs to find an affordable alternative.
