As part of our tobacco grant provided through Ohio Department of Health, we are assessing our community’s readiness for Lakewood. As an involved and dedicated member in the community, your input is very valuable. The results of the survey will be utilized as we plan and develop our programming and public health initiatives within this upcoming grant cycle. For completion of the survey, 1 participant will be selected to win a $25 gift card.
The link to the survey is below. There is a total of 25 questions that are required to be answered in order for the survey to be considered complete. Your responses should reflect upon your perception of your community’s readiness, not based on your own individual readiness. When prompted to answer, “What organization requested that you fill out this survey, “please select “Community Awareness and Prevention Association (CAPA).
