A medical professional making house calls? Yes, it’s back! Healthcare is evolving and nurse practitioners are at the forefront of the changes in healthcare delivery.

Ohio residents are taking advantage of a free health assessment provided by local nurse practitioners who are part of the 700 nurse practitioners nationwide with Matrix Medical Network.

Nurse practitioners visit to conduct thorough routine checkups and preventative screenings in the comfort of a resident’s own home. Home health visits can save members money too, as they prevent costly emergency room visits and ambulance rides.

All of the nurse practitioners have graduate and undergraduate nursing degrees and hold a Registered Nurse license and/or certification. Nurse practitioners are qualified to perform approximately 60 to 80 percent of primary and preventive care services as those provided by a doctor.

What to Expect
During a visit from a nurse practitioner, participating health plan members receive the following:

• Unrushed sessions in your own home to discuss and ask questions about any health concerns,
• A complete review of medicines and related instructions on how to take them as prescribed,
• A comprehensive health summary that can be shared with your primary care physician, and
• Information about community resources and programs available to you nearby.

In Ohio

• There are 120,000 health plan members eligible for this service – free of charge.
• In 2014, more than 30,000 health plan members participated in a health assessment.
• Participating health plans offer home health visits for older adults and other eligible plan members at no additional cost.
