Your feedback can improve the health of our communities

The Healthy Columbia Willamette Collaborative is conducting a 10-minute survey for people who live in Clackamas County. 

Healthy Columbia Willamette Collaborative is an alliance of our region’s hospitals and health systems that collaborate around understanding community health needs. This allows these health systems to leverage their collective resources to improve the health of our communities and the quality of services they provide.

The survey will be open until October 28 and will take approximately 10 minutes for you to complete. It is available in 11 languages.

There are no right or wrong answers. It’s your experience that matters! We encourage you to share this survey link with others in your community, so we can have the full picture of health needs in our communities.

Filling out the survey is your choice – it is completely voluntary. All answers are anonymous and not connected to your name. Your individual answers will not be released, nor will they be shared with HCWC partners, the public, or anyone else. Information shared with HCWC and the public will only be a combined summary of responses. We will not be able to identify the people who took the survey.

A CHNA is a report that identifies the key health needs, issues, and strengths of a community. It is developed by looking at data and asking community members for their input. Your voice and experiences are critical in helping us describe what is going on in the four-county region. 

Information from this survey will be used to inform community health improvement plans and how money is invested in community health and well-being. Some examples may be: 

• If a CHNA shows that community members require more accessible health services, including language interpreters, a hospital or health system may expand translation services
• If communities highlight housing as one of its greatest needs, a health system may invest in an affordable housing project

Therefore, this survey is an important opportunity to hear from communities what they would want to see in place to support health for everyone. The CHNA report will be available and accessible to the community upon completion of the project in Fall 2022. Learn more about the Community Health Needs Assessment.
