Free Chronic Disease Management Program Coming To Elkins Park

The KleinLife: Montgomery County Senior Center is hosting the free, six-week program that will help those with chronic illnesses manage.

ELKINS PARK, PA — The KleinLife: Montgomery County Senior Center in Elkins Park is getting ready to host a free chronic disease self-management program.

The program, titled "My Life, My Health," will run for six weeks, starting Wednesday, Jan. 3, 2024.

Each session is from 12:30 p.m. to 2:30 p.m. and uses an evidence-based approach

Program topics will include managing symptoms, working with your healthcare team, setting weekly goals, effective problem solving, how to relax and handle emotions, tips for eating and increasing activity, techniques to manage fatigue and pain, effective communication and getting a good night’s sleep.

The final session is planned for Wednesday, Feb. 7, 2024.

"People with different chronic health issues will attend together and through mutual support participants will build confidence in their ability to manage their health and maintain active and fulfilling lives," Gail Korostoff, director of the senior center, said.

KleinLife: Montgomery County Senior Center is located at 7763 Old York Road in Elkins Park

While the program is free, participants must register in advance by calling Korostoff at 215-635-5244.
