A proud member of the Marquis Health Services family, New Eastwood is a Subacute Rehabilitation and Skilled Nursing Facility in the heart of Northampton County conveniently adjacent to Easton Hospital, and an integral part of the Easton community for decades.

A solution driven Direct Admission service, New Eastwood’s Urgent SNF™ program provides an alternative option to individuals not meeting acute admission criteria, yet requiring or benefiting from skilled nursing supervision and stabilization. To minimize patient relapse upon return home, the program provides post discharge follow-up and support by New Eastwood’s clinical team

Designed for patients with diagnoses that include uncontrolled diabetes, exacerbation of COPD, ambulation dysfunction, exacerbation of CHF, dialysis, change in mental status or simply a health setback, New Eastwood’s Urgent SNF™ is also the perfect place to transition following a visit to the ER – as in the case of Gerhard Harms.

“While I wasn’t sick enough to be admitted to the hospital, I was not ready to return home safely”, states Mr. Harms. “The Urgent SNF™ Program at New Eastwood was presented to me by the hospital staff, and I am glad I agreed to take this route. The nurses were very caring and the Pulmonary Rehabilitation was an additional benefit to me”.
