The Bucks-Chester-Montgomery Link is bringing a pilot program to Montco SAAC, Norristown, to educate consumers on the risks of care transitions. Staff will be making presentations and distributing zip drives with an attached clip to go on a key-chains. They will give vital information if you find yourself in the hospital. We believe that educating and empowering consumers to take charge of their own care transition is a necessary step to ensuring that when you get home – you stay home.
