A PIAA Comprehensive Physical is mandatory for participation in OLSH Athletics. It is important to obtain the necessary forms before the physical. Click here to download the sports physical packet. Forms are also available from a coach and in the main office.
Students should arrive between 12:30-1 p.m. and the doctor will examine in order of arrival.
If students are participating in a fall sport, all physicals must be completed and turned in by Aug. 15.
