Join in an RN-led candid conversation on Advance Care Planning: how to make your own decisions about your future health care, and learn ways to talk with your family, friends, and physicians about health care choices for yourself and your loved ones. Sample Advance Directive forms will be available for you to take home.
Topics will include: What does it mean to be on a ventilator, or to have a tube feed? What’s is the difference between palliative care and Hospice care? What does it mean to be a Full Code versus Do Not Resuscitate? Q&A time will be provided.
