St. Alphonsus School is holding its annual registration day for new students preschool through eighth grade at the school on 2/20/12. Â
Parents registering their children on February 20 must bring the original, plus a copy, of the following documents: student birth certificate, baptismal certificate, social security card. Also needed for registration is a just a copy of the student’s immunization record and any custody papers. (No copies will be made at the school.)   Â
Depending upon the grade the child is entering, parents may receive a physician and dentist report form at registration to submit to their doctors and return to school. If the student is transferring from another school, a request for a copy of their report card and school records will need to be completed. Registration will be complete once all forms are submitted and the non-refundable fee of $75 per first child and $50 for each additional child is paid.
