If you have been impacted by remnants of Tropical Storm Fred, the individual assistance form (linked below) may be completed using the data sheets, also linked below. The forms are used to gauge the amount of loss in the region and are not a guarantee of a recovery amount, if any. Our role is to collect the data and forward it to the County Emergency Management Team.
