Health Inspections: Violations at Warren Eateries
The Health Department reportedly found food service violations at three Warren locations in May.

Bebop Burrito, 40 Market St., Warren — The Health Department noted 2 violations during an inspection May 9.

- A accumulation of water was observed on the basement floor in the sump pump area. Plumbing service must be called to address this problem. No food items or equipment were in contact with the water. Please fax/e-mail inspector will results of service.
- The floor in the kitchen is worn, exposing wood, it is not smooth and easily cleanable. Materials for floor and wall surfaces shall be smooth, durable, easily cleanable, and nonabsorbent for areas subject to moisture such as food preparation areas, walk-in refrigerators, warewashing areas, and toilet rooms. Per the person in charge, a contractor has been contacted to renovate worn portions of the floor to be smooth, durable, and easily cleanable Due to be fixed on May 12, 2013.
Fat Belly's, 632 Metacom Ave., Warren — The Health Department noted 11 violations during an inspection May 6.
- Hand washing signs not available in employee bathroom and men's room.
- The slicer had an accumulation of food and debris. The food contact surfaces of equipment must be clean to sight and touch. slicer was cleaned during inspection.
- Pasta, soup and chowder which were prepared in advance, were not cooled to 70 degrees in 2 hours and then to 41 degrees F within 4 hours. All items had been prepared day or two before and temped between 45-49 degrees. All items disposed of.
- Some items located in the refrigerator, were not marked with the date of preparation. Ready-to-eat potentially hazardous food , prepared in the establishment and held for more than 24 hours, must be marked with the date of preparation.
- The consumer advisory provided does not disclose the foods on the menu that are raw or undercooked. New menus are in the process of being made.
- Quarternary ammonium sanitizer solution was in excess of 400+ ppm in the sanitizer buckets. A quarternary ammonia sanitizer solution applied to food contact surfaces must be applied at the strength as indicated on the manufacturers label. Buckets were emptied, and filled from sink that had 200ppm reading.
- Proper cooling methods were not used for potentially hazardous foods. Per interview of staff proper cooling methods are not being followed, items are placed in ice bath right off of stove, in containers greater than 4" deep. Temps are not being taken during the cooling process. Hand outs for cooling were given and charts will be required to be kept on cooled items and must be available to inspector at re-inspection.
- Frozen liquid eggs cartons were observed thawing in the sink. Potentially hazardous foods must be thawed in one of the following ways: under refrigeration (41 degrees F or less), completely submerged under running water, as part of the cooking process, or in the microwave if cooked immediately after thawing.
- The facility is lacking test strips to measure the concentration of the quaternary sanitizing solution for the 3-bay sink.
- The nonfood-contact surfaces of the hood/vent system have an accumulation of dust, and greases.
- The nonfood-contact surfaces of the ice machine had an accumulation of debris.
Warren Skilled Nursing and Rehabilitation, 642 Metacom Ave., Warren — The Health Department noted 4 violations during an inspection May 9.

- Egg salad, cooked hot dogs, Pasta salad, Sauce, and stuffed peppers) were held at (43-44) degrees. Potentially hazardous food must be held at 41 degrees F. or below except during preparation, cooking or cooling. Items were voluntarily disposed of.
- Utensils (forks and spoons) are displayed with the lip surface up. Utensils are to be stored so that only the handles are touched by employees and consumers if consumer self-service is provided.
- Walk-in cooler has and ambient temp of 44 degrees, temp was checked multiple times during inspection and stayed at 44 degrees. Service needs to be called for this unit, the unit must be able to hold 41 degrees or below. No PHF's will be stored in unit until it is adjusted.
- The outside dumpster is not covered. Receptacles and waste handling units for refuse, recyclables and returnables shall be kept covered.
The Health Department also conducted inspections at Burger King and Federal Hill Pizza Company, finding no violations.
