2. Matthew Maynard, 33, of Burrillville, was killed on Oct. 21 at Greenville Ready Mix in Smithfield. Maynard was performing maintenance on a cement truck when he got caught in the truck's hopper. The family has man questions about this tragedy. (WJAR)
