CityServe’s Care Team provides compassionate, wraparound care coordination for the most vulnerable in our community. We encompass the social determinants of health by stabilizing lives in the areas of basic needs, financial stability, social support, physical health, emotional health, and vocational support. By partnering with churches, volunteers, government and community organizations, we support neighbors in crisis and build pathways toward long-term stability and self-reliance.
