Ashburn Health Inspections: All American Steakhouse, Senor Tequila's

The Loudoun County Health Department recorded violations at several facilities around the Ashburn area during recent inspections.

ASHBURN, VA — Several facilities in the Ashburn area recorded violations during health department inspections between Oct. 4 and Oct. 10.

COVID-19 restrictions around the state have disappeared, so health inspectors conduct most restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

    • Date inspected: Oct. 4
    • Total violations: 1
      • Kitchen manager could not locate proper documentation of an employee health policy showing kitchen staff are properly trained to report symptoms of illnesses transmittable through food. Manager must document staff training.
  • Trailside Middle School, 20325 Claiborne Pkwy Ashburn, VA 20147
    • Date inspected: Oct. 4
    • Total violations: 1
      • Ice machine observed with accumulation of black residues. Clean the surface at a frequency necessary to preclude accumulation of soil residues.
  • Siam Spice Thai Cuisine, 22330 S. Sterling Blvd A129 #129 Sterling, VA 20164
    • Date inspected: Oct. 4
    • Total violations: 4
      • Cold Holding unit in back of house and Half and Half mini fridge at the front of house do not have a method of verifying ambient air temperature.
      • Ice Used as Exterior Coolant, Prohibited as Ingredient. Observation: Ice used as a medium for cooling the exterior surfaces of Milk.
      • Observation: Observed rice scoops in stagnant water and tongs on oven handles, as well as tea scoop in a cup of water.
      • Slight accumulation of debris on floor near hot water heater and staining on ceiling tiles.

    • Date inspected: Oct. 4
    • Total violations: 7
      • The hand sink located in the back portion of the kitchen was observed blocked by a stainless steel container. Hand sinks that are utilized by food employees shall be used for no other purpose than hand washing and be kept free of any obstacles to encourage frequent hand washing. Employee removed the container.
      • 2 gallon jugs of milk (one in walk-in cooler, one in reach-in cooler at the front bar) were found with an expiration date of 10/02/2023. Fluid milk and milk products that are served/used must be Grade A. Milk that has surpassed the expiration date is no longer Grade A and must not be used. Employee voluntarily discarded the two gallons of MIlk.
      • EHS observed soil debris on the deli slicer. Food contact surfaces shall be clean to sight and touch. Clean the deli slicer so that there is no debris.
      • Tomatillo, Ancho and Verde sauces next to the grill were temped at 122-125F, outside of proper hot-holding temperatures. When hot-holding foods that are time/temperature control for safety (TCS) and are ready to eat (RTE), they must be held at 135F or more. Chef had employee reheat the sauces.
      • Two spray bottles containing a disinfectant were observed with no identifying label/information. When taking toxic materials/cleaners from bulk supplies, the common name of the material must be labeled on the working container. Chef labeled the spray bottles during the inspection.
      • A large pan of prepared cooked chicken was observed stored directly on the floor of the walk-in freezer. Food shall be protected from sources of contamination by storing foods at least 6" off the floor. Chef moved the container of chicken to a shelf within the freezer.
      • 2 mops were found stored after use in the basin of a mop bucket and in the basin of the mop sink. Mop heads should be hung or otherwise stored so that they may properly dry. Hang mop heads when they are not in use. Mop heads were hung to dry.

      • Facility has an approved variance to regulate the pizza sauce sitting at room temperature longer than 4 hours. This variance required the sauce to be labeled with date and time it is put out which has not occured.

    • Date inspected: Oct. 5
    • Total violations: 8
      • There are several employees present that have not been informed of their responsibility to report illnesses to management that relate to food. No signed employee illness reporting agreements on file for these employees.
      • Food handler observed touching butter with the top of their bare hand while scooping butter to portion into smaller containers. Food shall not come in contact with bare hands.
      • Employee observed dumping bucket of sanitizer into hand sink closest to kitchen entrance. Hand sinks shall be used for no other purpose.
      • The ice machine interior deflector is observed with buildup of pink residue. Food contact surfaces shall be clean and maintained clean.
      • Chlorine sanitize dispensed at dish machine tested at 25ppm. Proper range for chlorine sanitizer is 50-100ppm. Quat sanitize dispensed at 3-compartment sink tested at 0ppm. Proper concentration not met at 200ppm. Wares not sanitized.
      • Two uncovered pans of ribs in the cooling stage stored in walk-in-cooler on bottom shelf beneath shelving containing boxes of food and other food items. Food shall be stored in a protective manner, store uncovered cooling foods on top shelf away from potential contamination.
      • The following areas of the physical facilities in need of detail cleaning: The floor in the storage area near the grease tanks and the walls throughout the kitchen.
      • Observed part of the ceiling track, a ceiling tile and one of the ceiling vents dangling down from ceiling track nearest the dish room. The physical facilities shall be maintained repaired.

      • No Ambient Air Thermometer is inside of the Danby 1 DR Reach in.
      • The facility does not have appropriate test strips to monitor the concentration of the Quat sanitizer at the three basin sink. Test strips at site expired "May 2022." Facility does not have means of verifying internal temperature of Jackson High Temp Dishwashing Machine.
      • Observed Ice Machine's Guard inside machine not clean and has black residue.

    • Total violations: 3
      • A box of cabbage was observed stored directly on the floor in a cardboard box under the hand sink by the 3 VAT. Foods shall be stored at least 6" from the floor so that contamination from the premises does not occur. Move vegetables off the floor, or store on risers that elevate foods off the floor.
      • Observed in use tongs stored on oven handles and a scoop in a near empty container containing a shallow amount of water and a stray mushroom.
      • Observed the ceiling tiles to have stains above the 3 VAT and several areas, such as the walls by the hot water heater and the piping/caulking near the hand sink by the grill.

    • Date inspected: Oct. 5
    • Total violations: 5
      • Observed marinated raw beef stored over ready to eat food in walk in cooler. Prevent potential cross contamination by not storing raw animal foods over ready to eat foods.
      • There is no available Chlorine sanitizer in bar dish machine. Sanitizer is reading 0 parts per million. Maintain 50-100 parts per million in a dish machine using Chlorine sanitizer for proper sanitization.
      • Observed time/temperature control for safety foods (Tamale) not properly dated for disposition. Foods not labeled include: X. Ready to eat TCS foods held over 24 hours must be date marked with either made on date or date the food item must be consumed not to exceed 7 days.
      • Chicken tender stored in grocery shopping bag. Food contact surfaces of the equipment should be easily cleanable.
      • Dish machine is out of detergent. The malfunctioning audible alarm might require repair/replacement.

      • Pico, lettuce, and sliced tomato cold holding at improper temperature at prep cooler. Time/temperature control for safety (TCS) food should be cold held at 41 degrees or below.

      • Bartender observed washing their hands at a dedicated dump sink at the bar. Employees shall wash their hands at designated hand sinks only.
      • EHS observed tongs improperly used to place raw chicken on the flat top grill stored in a container of ready to eat bacon. Tongs also used with ready to eat bacon.
      • Cut tomatoes (50) and cut lettuce (51F) cold holding at improper temperatures at the prep cooler on the line. The large containers used for storing the tomatoes and lettuce were holding above the cold well.

      • Observed spices and cooking oil stored directly on floor in kitchen and in dry storage area. Food shall be stored in protective manner, elevate at least 6" off of floor.
      • Observed several ceiling light fixtures light shields not in place throughout kitchen. One shield observed severely cracked. Light bulbs shall be shatterproof or properly shielded.

      • Employee observed eating while chopping lettuce. Food employees may only eat, drink, or use tobacco in a designated area like a dining room or office area. Employees shall not eat or drink in the kitchen areas, warewashing areas, or storage areas where items may become contaminated.
      • There is no available Chlorine sanitizer in dish machine. Sanitizer is reading 0 parts per million. Maintain 50-100 parts per million in a dish machine using Chlorine sanitizer for proper sanitization.
      • Ceiling light bulbs aren't shielded in the walk-in cooler.
