Ashburn Health Inspections: Cold Stone Creamery, Home Plate
The Loudoun County Health Department recorded violations at several facilities around the Ashburn area during recent inspections.
ASHBURN, VA — Several facilities around Ashburn recorded violations during health department inspections from Aug. 16 to Aug. 23.
Most COVID-19 restrictions around the state have relaxed, so health inspectors are returning to conduct some restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection

- There was no variable employee health policy that employees have been trained on the signs, symptoms, and exposures of foodborne illnesses and whom to report to.
- Observed pizza that was purchased from another retail business being sold to customers at facility.
- Observed tapioca on shelf 76F (discarded), beef bulgogi 127F (reheated 193F), chicken 127F (reheated 179F), chili 119F (reheated 201F) at an improper hot holding temperature.
- Observed kimchi in facility, and EHS asked how the kimchi is made and the person in charge stated that the kimchi is out of temperature overnight.
- Observed chicken submerged in water in prep sink without running water.
- Observed in walk in freezer frozen fries being stored on the floor.

- The hand sink between the three basin sink and the hot holding line has no cleanser/soap.
- 2 knives that were stored on a magnetic strip above the prep area were observed with food debris on them.
- Several containers of salsa and a container of pre-assembled chimichangas observed with no date marking.
- A spray container of degreaser was found with no identifying label.
- 2 cans of spray aerosol pesticide were found on a shelf beneath the front counter.
- Several single use containers originally labeled and used to store peeled garlic observed being reused to hold hot sauces, salsas and other RTE food items.

- The facility repeatedly is unable to provide verifiable means that employees have been trained in the signs and symptoms of food borne illness and how to report them.
- A large presence of flies was observed within the facility.
- The EHS observed an employee's personal phone on the counter next to ready to eat (RTE) waffle cones that were being dipped in chocolate.
- EHS observed clean, wet wiping cloths air-drying/hanging above the three basin sink, and above the sanitized drying area for dishes.
- The facility has severe buildup of soil debris (food, syrup, sugars) on floors, walls and surfaces of equipment in all areas behind the front counter and the kitchen area. Hand washing sink also has sever debris along it's sides and basin.

- Total violations: 4
- Feta cheese, provolone, cut lettuce and cut tomato were found cold holding at improper temperature. The cooler is not maintaining temperatures of 41F or below.
- Meatballs, pasta, cooked chicken, and cut tomatoes were found with no date marking.
- Several spray containers with various colored liquids were found throughout the facility with no identifying information.
- Several single-use containers were observed washed and stored for re-use.

- Bartender observed dispensing RTE big rock ice from mold using bare hands. Hands touched ice for consumption.
- Improper cooling methods used to cool chicken stock and corn stock in walk in coolers. Stock stored in large deep containers.
- Temperature gauges at dish machines located in the Audubon and Chandler Barn bars are in disrepair, temperature needle is stuck.

- The low temperature dish machine located inside the main bar, as well as the low temperature dish machine behind the lounge bar is not dispensing sanitizer. Sanitizer measured at 0PPM, both dish washers.
- A large container of Gumbo was found in the walk-in cooler with a date marking of 7/27/22.
- 2 spray bottles at the main bar were found with a clear liquid inside them and no identifying label.
- A container of raw shrimp was observed thawing in a container of stagnant water.
- Several spatulas in use on the main line were observed stored in between uses in a container of stagnant water.
- The crushed ice machine was observed to have moderate accumulation of black & pink soil debris on the deflector.

- A container of food belonging to an employee was found stored with food intended for guests. (Container of soup next to egg patties in the reach-in cooler).
- This facility uses Quat to sanitize, but did not have testing strips or a device to measure the concentration of sanitizer.

- Observed no paper towels available at the Hand Sink in the back by the 3 VAT sink.
- Observed soda nozzles to have a black mold like substance when tested with an alcohol swab. Minute Maid Lemonade/Water/Sprite were checked.
- Observed wet mop in the bottom of the bucket component of the mop bucket not allowing for proper air drying.

- Facility offers salmon on the menu and cooks it to the request of the guest (will serve undercooked if requested). Parasite destruction documentation for the farm raised salmon is not maintained on file.
- TCS (time temperature control for safety) foods (cut tomatoes, cut lettuce, sliced turkey) are cold holding at improper temperatures at the line prep table cooler)
- A review of the menu with the foodservice operator indicates that there is no consumer advisory for the following food item(s) that may be served raw and/or undercooked: burgers, undercooked eggs, and salmon.
- Packaged food (salads, sandwiches) offered for sale in a "grab and go" cooler at the front desk area. The packaged food labels are only labeled with common name ingredients. Net wt statement, allergen statement, and sub-ingredients are all missing from food labels.
- EHS Observed fan with heavily soiled blades/cover (dust and dirt) sitting right next to uncovered bowl of flour (breading) for fry station. .
- EHS observed knives stored in between prep tables and also on wooden magnetic knife holders that are visibly very soiled.

- Egg patties (65F) noted as improperly cooled to prevent the growth of harmful bacteria. The person in charge (PIC) stated that they prepared the eggs at 5 am and let them sit at room temperature for several hours before placing them in the walk in cooler.

- The facility has two high temperature dish machines and does not have a irreversible temperature indicator to monitor the utensil temperature of the dish machine.

- The handwashing sink is observed being used for thawing food.
- Chemical dishwasher in bar area observed without sanitizer. As per the PIC, the facility does not have more sanitizer to replace the empty container.
- Raw burger patties and raw chicken registered at 44°F. Said unit registered at 38°F. As per the PIC the top section of said unit was left open for pepping during lunch rush.
- Cheese, sausages and ham in top section of pizza prep refrigeration unit registered between 44 to 48°F. Said unit registered operating at 36°F. As per the PIC the top section of said unit was left open to prepping pizza during lunch rush.
- Ventilation hood observed with heavy accumulation of grease and dust.
