Ashburn Health Inspections: Don's Woodfired Pizza, T Zone Dessert Bar
The Loudoun County Health Department recorded violations at several restaurants around the Ashburn area during recent inspections.
ASHBURN, VA — Several facilities in the Ashburn area recorded violations during health department inspections between Nov. 15 and Nov. 21.
COVID-19 restrictions around the state have disappeared, so health inspectors conduct most restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

- Total violations: 2
- The low temperature dish machine registered 0ppm chlorine when tested. Food contact surfaces must be sanitized before use, after cleaning. EHS determined that the chlorine sanitizer was empty. Person in charge (PIC) installed a new container and primed the machine. The dish machine is now registering 50 ppm chlorine.
- A container of Goat Korma with a date marking of 11/05 and a container of Alo bhujia with a date mark of 11/04 were observed in the walk-in cooler. Time/temperature control for safety foods (TCS) that are ready to eat (RTE) must be consumed, sold, or discarded after 7 days from the date that the food item was opened/prepared. PIC voluntarily discarded both containers of food.

- Inside of the ice machine observed with accumulation of black residues.

- A container of sanitizer tablets is stored on a shelf above an open container of food. Store chemicals below and away from food and clean utensils. Corrected on site by the Person in Charge relocating the container of sanitizer tablets.
- The chlorine test strips expired in September 2023. Obtain and use non-expired test strips. After this item was noted the Person in Charge directed a team member to order test strips for delivery tomorrow.
- Some floor tiles are missing and/or broken in then vicinity of the hand sink. Repair and/or replace the missing and/or broken floor tiles as appropriate.

- Observed raw shrimp stored over ready to eat food in cooler. Prevent potential cross contamination by not storing raw animal foods over ready to eat foods.
- Observed deli meat, corn beef cold holding at improper temperature (47F) at the drawer cooler below the flat top. Time/temperature control for safety (TCS) foods must be held at 41F or below to prevent growth of harmful bacteria.
- The concentration of the veggie solution was measured outside the acceptable range.
- Sanitizer solution at the three compartment sink in the bar observed to be too strong.

- Raw chicken observed improperly stored on a cart in the walk in over fully cooked tamales. Raw chicken also observed improperly stored over fully cooked sausage on the speed rack in the walk in cooler.
- The horchata drink dispenser is not maintaining the milk containing horchata at proper cold holding temperatures. Horchata cold holding at 57F which is above the required minimum of 41F. Cut tomatoes 50F cold holding at improper temperatures at the self service salsa bar.
- Two chemical spray bottles observed without a label indicating what they are filled with.

- Two ceiling tiles in women's bathroom observed with water damage.
- Observed quat sanitizer solution at the 3-compartment sink at 0ppm. Food contact surfaces shall be sanitized before use after cleaning. Quat sanitizer solution shall be provided at proper concentration (200ppm).
- Observed front area hand sink used for dumping bucket of sanitizer. Front area hand sink observed with wet cloth stored in sink basin and back area hand sink observed with large chunks of ice stored in basin. Hand sinks shall be used for no other purpose.
- Observed employee touching ready to eat sliced tomato with bare hands. Employees shall not contact exposed ready to eat foods with their bare hands.

- Cut melon (50F) cold holding at improper temperatures in the small countertop homestyle refrigerator.
- The person in charge in the kitchen is not wearing a hair restraint nor is their hair tied back away from their face.

- The quat test strips expired in October 2022 and the chlorine test strips expired in April 2021. Obtain and use non-expired test strips. Person in Charge stated that they would obtain fresh test strips with the next food delivery.

- Total violations: 8
- Duties of the person in charge are not being performed. Temperature abuse of time/temperature control for safety foods (TCS) that are ready to eat (RTE) is repeatedly observed in the reach-in cooler at the entrance to the kitchen. Date marking is not performed. The person in charge shall ensure that foods are not temperature abused and that proper date marking practices are followed.
- No written policy or procedure for the clean up of vomit or diarrhea was located or was able to be provided. A written policy or procedure for the proper clean up of vomit or diarrhea must be provided. EHS emailed person in charge (PIC) a copy of the NOROVIRUS clean-up worksheet.
- Pork buns (10) and BBQ Buns (6) were observed being offered for sale in the open faced deli cooler and the closed cooler on the counter. Establishment management is aware that the buns are not from an approved food source. Food shall be obtained from sources that comply with law. Employee voluntarily discarded and denatured all of the buns during the inspection.
- The food contact surfaces of the grinder that is located on the counter was observed with heavy food debris. Food contact surfaces of equipment and utensils shall be clean to sight and touch. Clean the grinder.
- Boba bubbles found hot holding at improper temperature (106F). Boba tea bubbles must be held at 135F or more once rehydrated. Hold Boba Bubbles at 135F or more.
- The reach in cooler located directly to the right as you enter the kitchen space is not maintaining foods at 41F or less. TCS food items cold holding at improper temperature (4 containers of sliced papaya, 3 containers of sliced papaya and carrots in a vinegar solution 46F, one open basket of sliced papaya 47F, one full tray of viet crepes 47F and a small container of reconstituted rice noodles 46F, cut mango, guava and dragon fruit (49F, 51F, 50F). TCS foods that are RTE must be maintained at 41F or less. This cooler has a history of improperly cold holding TCS foods and should not be used until it is able to maintain foods at 41F or less, Employee voluntarily discarded all TCS foods during the inspection.
- No date marking is being performed within the facility. When keeping TCS RTE foods longer than 24 hours, a date marking the day that the food was prepared/opened door a discard date not to exceed 7 days from the date that the food item was opened/prepared. Date mark all TCS RTE foods items (sliced papaya, crepe batter, Vietnamese crepes, rice noodles once they have been reconstituted, and milk based additive that is moved to another container other than its original container.
- An aerosol spray can of Raid insecticide was found within the kitchen. This insecticide is not approved for use in kitchens. A restricted pesticide shall be applied only by an applicator that is certified, or a person under the direct supervision of a certified applicator. Do not use spray insecticides within the kitchen.

- The facility does not have a written policy for the clean-up of vomit or diarrheal events. A written policy that outlines the proper clean-up of vomit or diarrhea must be available for employees to follow. EHS emailed a copy of the NOROVIRUS worksheet. Print this out and make it accessible to employees.
- An open coffee mug with warm coffee inside was observed stored on a shelf with food products. Employees may drink in food areas if the drink container is closed (lid) and does not constitute a contamination risk to foods or food contact surfaces.
- EHS observed open bags of meatballs, baked chicken breasts, portioned shaved chicken breast with no date marking. When keeping time/temperature control for safety (TCS)foods that are ready to eat (RTE) longer than 24 hours, a date marking when the food was opened/prepared or a discard date not to exceed 7 days from the date the food was opened/prepared must be labeled on the food/container. Date mark these items.
- EHS observed several containers of cleaning chemicals stored on the top shelf in the dish washing area, stored over food contact surfaces of equipment and utensils. Cleaning supplies and other toxics must be separated from food/f When keeping time/temperature control for safety (TCS)foods that are ready to eat (RTE) longer than 24 hours, a date marking when the food was opened/prepared or a discard date not to exceed 7 days from the date the food was opened/prepared must be labeled on the food/container. Date mark these items.
- Several knives were observed dirty and stored underneath the convection oven located in the prep area in the back kitchen. Utensils that are in-use, but stored in between uses must be stored on a clean dry surface. Knives were moved to the three basin sink to be washed.
- Chlorine test strips to monitor the sanitizer levels of the low temperature dish machine have been compromised (wet - no longer viable). Test strips or other testing device to monitor the concentration level of sanitizer must be available. Procure chlorine test strips.

- Milk registered between 46°F to 50°F on one door low boy refrigerator in second floor. As per the PIC said milk were set out during breakfast. Also said unit registered operating at 50°F.

- The lid of the low boy refrigerator holding cheese and cold cuts in main kitchen observed broken.

- Observed cute lettuce and sliced tomato cold holding at improper temperature (57F). Time/temperature control for safety (TCS) foods must be held at 41F or below to prevent growth of harmful bacteria.
- Medicines not necessary for the health of the employees are stored on the shelf above the kitchen counter.
- The ceiling light bulbs in the kitchen are not shielded, coated, or otherwise shatter-resistant.

- Management, food employees or conditional employees were not aware or informed of their responsibilities in reporting symptoms or diagnosis of disease transmissible through food.

- Total violations: 4
- A spray bottle containing a blue liquid with no identifying label was found stored at the front counter. When toxic materials (cleaners) are taken from bulk supplies, the common name of the material must be labeled on the working container. Label the spray bottle with the common name of its contents.
- Raw sausage was observed stored over a container of cooked sausage, raw meatloaf mix was found stored over raw salmon. Food shall be protected from cross-contamination by separating raw proteins from ready to eat (RTE) foods, and separating raw proteins from one another by arranging from lowest internal cook temperature (Salmon 145F) to highest internal Cook temperature (sausage/meatloaf 155F). PIC rearranged those proteins so that they are stored correctly.
- The soap dispenser at the hand sink located in the dish washing area is not dispensing soap. All hand sinks must have a cleanser for washing hands at all hand sinks that are utilized by employees. Management repaired the soap dispenser.
- When requested, copies of the employee health reporting policy were not available for review. Verifiable means that employees have been trained in the signs and symptoms of food borne illness and how to properly report them must be available for review when requested. EHS emailed a copy of form 1B in both English and Spanish. Make copies of this form, have employees read & sign and have available within the facility at all times.
