Ashburn Health Inspections: Fruit Flies, Dozens Of Dead Roaches
The Loudoun County Health Department found violations at several restaurants around Ashburn and Sterling during recent inspections.
ASHBURN, VA — Several facilities around Ashburn recorded violations during health department inspections from Oct. 19 to Oct. 25.
Most COVID-19 restrictions around the state have relaxed, so health inspectors are returning to conduct some restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

- Observed in use knives stored in between prep tables. Observed ice bin to have large accumulation of mold like substance throughout.
- An unmarked spray bottle full of bleach was found by the cook line.
- Thermometer in the Freezer Walk in not functioning.
- Observed holes/openings/white light under door in back of house and screen door is not sealed either/has holes in screen. Could allow for possible entry of pests.
- Observed in use tongs on oven and knives in-between prep table.
- Observed washed pots and pans air drying on rack upside down with water pooled in various dips and depressions of handles/ other parts of the pots and pans preventing proper air drying.
- Bleach test strips for testing sanitizer at 3 compartment sink not available, no test disk/heat registering tape available to confirm temperature of dishwashing machine internally.
- Observed pants and a jacket on the hot water heater, keys on the line, various clothing on the dry storage racks. Observed various employee items throughout back of house (purses, cells phones, etc.)

- Observed the following items prepared in the last 24-48 hours intended to be kept for up to 72 hours not date labeled: Cooked Fish, Cooked Beans, Bean Stew, Okra Stew, and Cooked Spinach Stew.
- Observed the followed prepared items in unlabeled containers: Cooked Spinach Stew, Okra Stew, Bean Stew.
- Observed light to be visible under door. Door sweep not protecting outer opening to prevent possibility of pests entering.
- Observed several fruit flies throughout the kitchen.
- Observed Millets soaking in an uncovered large pot underneath the hand sink by the 3 vat sink on the floor.
- Employees observed working in the food service area without proper hair restraints. Observed staff in back of house preparing food with beards not covered by beard guard.
- Observed in use rice scoop stored on top of rice cooker and tongs stored under oven.

- Handwashing facility at the bar observed without paper towels. Said sink also observed being used at dump sink with strainer placed in it.
- Handwashing sinks near the wok and stoves in the kitchen and at the bar observed without employees must wash hands sign.
- Raw chicken and raw beef observed stored over ready-to-eat open container of fried pork on a shelf in the walk in refrigerator.
- 16 raw shell eggs in a carton registered at 65°F observed being held at room temperature on table next to the grill in kitchen. As per the PIC said eggs were taken from the walk in refrigerator and placed there about an hour ago for food prep.
- Cooked pork sauce registered at 56°F on spice cart at room temperature in front of the wok stove. As per the PIC said food item was placed at said location more than an hour ago.
- Several temperature control for food items such as cooked chicken wings, chicken curry, pork curry, fried pork and fried tofu registered between 44°F to 56°F in low boy refrigerators. As per the PIC said food items were taken from the walk in refrigerator, left outside on food prep table at room temperature for sometime (approximately 40 minutes) prior to placing them in the units and some units left open to while prepping to get ready for opening for lunch.
- Cut leafy greens registered at 60°F on food prep table in kitchen. As per the PIC said food items were taken from the walk in refrigerator and placed them on food prep table at room temperature an hour ago.
- Date marking not provided to several food items such as fried pork, pork sauce, beef broth in the walk in refrigerator. As per the PIC said food items were cooked and placed in said unit 1 to 2 days prior.
- Machine detergent container observed being reused for storing cooking sauce.
- Few live fruit flies and a dead roach observed in kitchen.
- In-use utensils improperly stored between use in that ice scoops observed stored with handles touching the ice.

- Observed light to be visible under door. Door sweep not protecting outer opening to prevent possibility of pests entering. Door is also propped open.

- A container of Cooked Potatoes with a date marking of 10/04, a container of yogurt dumplings with a date mark of 10/10, and two containers of cut & blanched fries with date markings of 10/13 were found in the walk-in cooler.
- An ice cream scoop and several stirring spoons were observed stored in a container of stagnant water at the beverage station.
- Single-use ice cream buckets have routinely been observed reused for other purposes in this establishment. Reuse of single use containers is not permitted.
- Only one operating bathroom in the facility. There is no covered trash can in the bathroom.

- Hand Soap at hand washing station in the back of house found to have unopened soap bottle on top shelf within arm's reach.
- Observed the following prepared items uncovered in cold holding storage: Macaroni and Cheese, Raw Chicken, Raw Salmon, Raw Beef, Cooked Beef Ribs, White Fish, and Cat Fish.
- Observed Fried Chicken at 70F under Line Utilizing Time as a Public Health control without documentation or time labels on the pan/chicken.
- Observed several fruit flies in the back of house.
- Observed light to be visible underneath the back door allowing for possible entry from pests.
- Observed Bleach Mixer at 3 VAT used to fill wiping cloth buckets and bucket containing sanitizer at 15ppm used to clean countertops in the main kitchen.
- Observed In Use Tongs stored on the oven handles.
- Observed Walk In Unit to have ambient temperature of 44F. Thermometer reflects 50F.
- Bleach test strips for testing sanitizer at 3 compartment sink not available.
- Observed several wet mops grouped together mop end down on the floor by the back area.

- The Person in Charge is not maintaining active managerial control of the facility and has not ensured that staff are trained in food safety as it relates to their assigned duties. Staff are not changing gloves and washing hands after touching or wiping their face, after handling soiled items, or after handling raw or undercooked animal food. Staff are not using soap when washing hands and washing from less than 10 seconds. Staff are handling ready to eat food with their bare hands. Staff are washing wares in a hand sink and not sanitizing them. Staff are not monitoring temperatures of incoming shipments. Kitchen staff do not have access to a functional food thermometer. Staff are not marking foods held using time as a public health control in accordance with the posted written procedures.
- When asked, the Person in Charge was not able to provide verifiable documentation that all on-duty staff have been trained on employee health policy.
- This food establishment has not established procedures for cleaning up vomiting or diarrheal events that happen in the facility.
- Staff are observed failing to wash their hands after handling raw animal food and soiled wares.
- Staff observed failing to use soap when washing their hands. Staff observed failing to lather for a minimum of 10 seconds when washing their hands.
- Staff observed using their bare hands to garnish a dish with sprouts.
- A shipment of wide rice noodles measured at 75°F was accepted by staff.
- Staff are failing to doff soiled gloves after engaging in activities such as handling soiled wares, touching or wiping their face, after handling raw animal foods.
- Partially cooked chicken stored above uncovered lettuce. Raw chicken stored on top of raw beef.
- A cut tomato was observed stored directly in contact with a shelf in a cooler.
- A staff member was observed rinsing pots in a hand sink and putting them away to be used without a sanitizing step.
- Foods such as rice noodles at 49°F, rice at 80°F, fried Thai basil at 80°F, bean sprouts at 44°F, peanut sauce at 43°F and green curry at 44°F cold holding at improper temperatures.
- Containers of foods held using time as a public health control are not marked to indicate a four hour window as specified on the written procedures.
- The chlorine solution in the three compartment sink testing above 200 parts per million does not meet the requirements of 21 CFR 178.1010.

- Hand soap not observed at the rear hand sink at the start of the inspection.
- The Person in Charge was not able to located the parasite destruction documentation for the sushi fish such as salmon and white tuna.
- Lo mein noodles at 47°F and 48°F greater than six hours after cooking inadequately cooled.
- Written procedures not available for foods held using time as a public health control. Some foods such as sushi rice and fried rice that are held on time control according to the Person in Charge are not marked with a four hour window.
- All items that consist of or contain raw or undercooked animal food are not disclosed on the menu. None of disclosures that are present on the menu are linked to the reminder footnote.
- A bag of onions is stored on the floor near the dishmachine.
- The walk in cooler floor is in poor condition.
- Non-food contact surfaces of various pieces of equipment including rice warmers, walk in cooler shelves, walk in cooler walls, and prep cooler lids are noted in need of cleaning.

- Hot water coming from the hot water heater for the establishment is not meeting current hourly demands. Hot water for the 3 basin sink was checked by EHS's department issued Mk4 thermapen 5 times during the inspection period. Maximum temperature registered at the 3 VAT sink was found to be 96F.
