Ashburn Health Inspections: Hundreds Of Mouse Droppings, Several Flies, Heavy Mold Growth

The Loudoun County Health Department recorded violations at several facilities around Ashburn during recent restaurant inspections.

ASHBURN, VA — Several facilities around Ashburn recorded violations during health department inspections from Oct. 5 to Oct. 11.

Most COVID-19 restrictions around the state have relaxed, so health inspectors are returning to conduct some restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

  • Pazzo Pomodoro, 19825 Belmont Chase #185 Ashburn, VA 20147
    • Date inspected: Oct. 5
    • Total violations: 5
      • Observed raw meats, soup, and desserts in prep cooler and walk in cooler uncovered.
      • Observed slicer with food debris on metal part at the bottom of the slicer near the blade.
      • Observed dishwasher with 0 ppm of chlorine sanitizer at beginning of inspection.
      • Observed a variety of foods on the floor of the walk in freezer.
      • Observed meat sauce with prep date of 9/25.
  • Brambleton Golf Snack Bar, 42180 Ryan Road Ashburn, VA 20148
    • Date inspected: Oct. 5
    • Total violations: 4
      • The food contact surfaces of the spatulas and knives stored on the magnetic strip are not clean to the sight & touch.
      • The interior of the ice bin is not maintained in a clean condition.
      • The sides of the freezer are noted in need of cleaning.
      • The cove base in the dish room is in poor condition and is not correctly attached to the wall. A floor tile under the freezer is in poor condition.

    • Total violations: 18
      • When asked, the Person in Charge was not able to provide verifiable documentation that all staff have been trained on employee health policy.
      • Paper towels not available at one of the kitchen hand sinks.
      • The milk with a sell-by date of yesterday is no longer compliant with grade A standards.
      • The Atlantic salmon is served cooked to order and there is no indication on the packaging that the fish is aquacultured or frozen for parasite prevention or destruction purposes.
      • Slider patties made from raw ground beef stored above raw fish.
      • The bar glass washer is not sanitizing.
      • Cheese sauce cooked at approximately 2:00 PM improperly cooling. The sauce was at 137°F, 86°F and 73°F at 4:30 PM.
      • Cheese sauce hot holding at improper temperatures - the portion of the food that is above the waterline of the double boiler is measured at 101°F,
      • Garlic butter stored on the counter measured at 70°F cold holding at improper temperatures.
      • Chicken salad observed date marked for an eight day holding period from 09/28-10/05.
      • All animal food items served undercooked are not disclosed by being asterisked to the reminder footnote (seared ahi tuna).
      • One step cooling of fresh tomato salsa conducted while the food is stored in a large portion in a tightly covered heavy plastic container in the walk-in cooler. Cheese sauce cooling in a large portion in a heavy plastic container stored on a counter.
      • Vacuum packaged fish is not removed from the sealed containers prior to thawing in accordance with the package labeling.
      • The facility has no means to verify that the high temperature dish machine is sanitizing.
      • Test kits are not available for each chemical sanitizer used in the facility.
      • There is no sanitizer being dispensed by the bar dish machine (sanitizer tested at 0 ppm and the container was empty) and there is no visual means to verify that sanitizers is being dispensed or visual or audible alarm to signal if sanitizer is not being delivered.
      • The interior ceiling of the ice machine bin is noted in need of cleaning.
      • There are multiple missing ceiling tiles in the kitchen in the vicinity of the small vent hood.

    • Date inspected: Oct. 5
    • Total violations: 9
      • When asked, the Person in Charge was not able to provide verifiable documentation that all on-duty staff have been trained on employee health policy - a form is not available for one employee.
      • Open drink containers observed.
      • A staff member was observed failing to wash their hands & change their gloves after handling raw shrimp.
      • Gluten free pizza crust observed contacting a surface that has not been cleaned & sanitized - the crusts have fallen out the the packaging and are touching the cooler shelf.
      • Foods such as marinara at 122°F and meatballs at 104°F hot holding at improper temperatures.
      • Multiple foods such as pizza cheese at 45°F, red sauce at 45°F, garlic bread at 67°F, garlic knots at 68°F, penne at 45°F, spaghetti at 45°F cold holding at improper temperatures.
      • Staff are not marking the actual date of preparation on foods prepared greater than 24 hours in advance - multiple foods in multiple coolers are marked for June 22.
      • When asked, the PIC was not able to locate written procedures for time as a public health control.
      • Cooling chopped ham is observed stored in the top of an open sandwich cooler.
  • Home2 Suites by Hilton, 43340 Defender Rd Chantilly, VA 20151
    • Date inspected: Oct. 5
    • Total violations: 3
      • The Person in Charge has not ensured that staff are trained in food safety as it relates to their assigned duties. The food service worker is not monitoring the temperature of the high temperature dish machine using the maximum temperature registering thermometer. The food service worker was not able to demonstrate correct monitoring the concentration of sanitizer.
      • When asked, the Person in Charge was not able to provide verifiable documentation that all on-duty staff with food and beverage responsibilities have been trained on employee health policy.
      • When asked, facility staff were not able to locate a written plan for responding to vomiting or diarrheal events.

      • Condiment containers stored on a shelf in the walk-in cooler below raw beef.

      • Not all employees had a verifiable FDA Form 1-B that employees have been train of the signs, symptoms, and exposures of foodborne illnesses and who to report to.
      • Observed a metal tray covering the hand sink in kitchen.
      • Observed no paper towels at hand sink by 3-compartment sink.
      • The blade of slicer had a build up of food debris on the blade.
      • In reach in cooler in bar area half and half 45F at an improper cold holding temperature.
      • Observed soup base and a bag of onions stored on the floor.
      • Observed wet cloths stored on prep cooler cutting broad. Corrective Action: Cloths in-use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer solution at a appropriate concentration. Person in charge placed in sanitizer bucket.
      • Observed the use of home depot buckets for storage of food broth in the walk in cooler.

      • Observed no Paper Towels by the Hand Sink at the front and the paper towel machine to be jammed at the Hand Sink in the back.
      • Observed several items not covered in a state of cold holding including chicha morada, Cooked Beans, Cooked Soups, Cooked Peppers. Observed Raw Beef stored over Ready to Eat Beans.
      • Container of food/juices that have been prepared and moved from original packaging/combined with other ingredients are found stored without common name labeled on foods.
      • Observed Rice Scoops in Stagnant Water, In Use Tongs Stored on the Oven Handle, and cup sitting in chicha morada used for serving.

    • Date inspected: Oct. 6
    • Total violations: 7
      • During the inspection the Environmental Health Specialist (EHS) observed a front line employee touching ready to eat (RTE) food (French Fries) while portioning them into cardboard containers for orders. Use of a utensil was observed but is not enough protection against bare hand contact with food.
      • Several utensils (whisk, spatula, scraper) were found stored with food debris on them.
      • Evidence of Rodents was observed within the kitchen and dry storage area's of the establishment. Harborage conditions were observed in the outdoor trash area, as well as inside the back door and the dry storage area.
      • Two personal cell phones and a tube of chapstick was observed stored on the line, in the vicinity of food preparation.
      • An employee on the line was observed with no hair covering.
      • EHS observed several white, 5 gallon buckets uncovered and full with oil/grease stored in them, and spillage of oil/grease on the ground in the outdoor refuse area. When asked the PIC said that the proper drums for storage until pick-up were full and unable to hold anymore.
      • EHS noted severe build-up of grease/oil/fats/soil residue on the inside, outside and joints/door tracks of the outside dumpster. Build-up is preventing the complete closure of the sliding door to deter rodents/wild animals/insects.

    • Date inspected: Oct. 6
    • Total violations: 1
      • Sausage patties (49F) cold holding at improper temperature at the open top prep cooler due to over stacking the pans.
  • Quickway Japanese Hibachi, 21031 Tripleseven Rd #180 Sterling, VA 20165

      • A knife with food debris was found stored on a magnetic strip above the prep table.
      • A container of imitation crab meat was found without date marking at the sushi station cooler (below).
      • Time as the public health control is used to manage the safety of the sushi rice. A written policy/log sheet is provided to track/log the time used for the sushi rice, but it has not been filled out in several days.
      • During the inspection it was noted that the walk-in cooler did not have adequate lighting to sufficiently view/identify food items on the shelves. The EHS performed a light test with a light meter roughly 4' from the ground, resulting in a grade of 5 foot candles.

      • Raw chicken and raw shrimp observed stored above cut vegetables in the shelf of walk in refrigerator.
      • Date marking not provided for cooked ready to eat (RTE) chickpeas in the reach in refrigerator in kitchen. As per the PIC said food were prepared a day prior and is held for few days.

      • Observed Cooked Sausage at 93F, and Chicken Patties at 105F from yesterday in a hot holding unit.
      • Observed Chicken Patties in Hot Holding Unit with Temperatures of 105F and 105F. Observed Sausage Patty Served at Line to Have a Temp of 93F.

      • Observed food items (House Buffett Sauce 9/28, pickled pepper 9/24, Tomato Soup 9/24, meatball sauce 9/28) that were kept past the maximum of seven days. Food items removed from package or prepared and held in a food establishment for more than 24 hours need to be date marked for maximum of 7 days.

      • Stacks of containers of raw chicken observed stored directly on the ground of the walk in cooler. Liquid from the raw chicken observed leaking from the boxes on to the floor.

    • Total violations: 9
      • Handwashing sink in the kitchen observed without soap.
      • Open can of tomato puree in the three door reach in refrigerator observed with heavy mold growth.
      • Chicken curry registered at 125°F in hot holding unit. Said food was left on the counter prior to placing them in the reach in hot holding unit.
      • Jackfruit biryani registered at 44°F in the lowest section of low boy refrigerator. Said section of said unit registered operating at 44°F. Said food was prepared 2 days prior and was held in said location.
      • Several live fruit flies observed in food prep areas in kitchen and in restroom.
      • Hundreds of of mice droppings observed throughout the kitchen. Mice dropping observed on shelves holding dry pots and pans, in boxes that would be used to hold ready to eat food items, on floor prep tables, on shelves and on floor wall junction.
      • Several dead fruit flies observed on glue traps and on floor, on boxes and on food prep tables throughout the facility.
      • The faucet for hot water observed leaking for handwashing sink.
      • Several unused old equipment observed dirty with debris, dust, mice droppings and dead flies in and on them.
