Ashburn Health Inspections: Live Roach, Dead Flies In Traps
The Loudoun County Health Department recorded violations at several facilities around the Ashburn area during recent inspections.
ASHBURN, VA — Several facilities in the Ashburn area recorded violations during health department inspections between March 15 and March 21.
Almost all COVID-19 restrictions around the state have relaxed, so health inspectors are returning to conduct some restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

- Plastic To-go bowls observed utilized as scoops for rice and flour in bulk storage do not have handles, thus barehand contact with ready to eat foods (RTE) is occurring. Utensils used to handle/dispense/measure RTE foods must protect from contamination of the food items. Discontinue use of bowls or other utensils that do not have handles. Handled Utensils may be stored within the foods with the handle up and not in contact with the food.
- The hand washing sink on the cook line was observed blocked by a basket stored within the basin. Hand washing sinks must be free of any obstruction and easily available for use to wash hands at all times. Basket was removed from the hand washing sink so that it was no longer obstructed. Corrected.
- Several boxes of vegetables observed stored directly on the floor in cardboard boxes in the back portion of the kitchen. Foods shall be stored at least 6" from the floor so that contamination from the premises does not occur. Move vegetables off the floor, or store on risers that elevate foods off the floor.

- Facility does not have test strip to measure the concentration of the sanitizer in that the sanitizer test strip provided at the facility has already changed color.
- Parallel Wine & Whiskey Bar, 43135 Broadlands Center Plaza #121 Ashburn, VA 20148
- Date inspected: March 15
- Total violations: 4
- Raw shell eggs observed stored improperly over ready to eat trays of marshmallows in the walk in cooler.
- Unwrapped trays of house made marshmallows observed in direct contact with the metal siding of the speed rack shelf they are stored on. Speed rack shelf is not a food contact surface. Food shall only be in contact with equipment and utensils that are clean and sanitized.
- TCS foods not date marked correctly. Some TCS foods are missing date marks (butter chicken sauce, rice) , others are date marked with the wrong month numbers (grit cakes 8/12)
- Whipped ricotta cheese spread observed held past the maximum 7 days at 41F or less. Said cheese spread date marked 3/2/2023. CHeese spread held for 13 days in the cooler.

- No Soap or drying provision is provided at the hand washing sink in the back portion of the kitchen. Hand washing sinks shall be equipped with a cleanser and hand drying provisions at all times to facilitate good hand washing practices of food employees. Provide soap and a hand drying provision.
- Raw chicken in the thawing cooler was found store directly on top of a open container of thawing, marinating beef. Raw animal proteins shall be separated and arranged in a way that prevents cross-contamination. Store raw proteins in order of lowest internal cook temperature to highest. Raw chicken was rearranged so that the raw chicken was stored below the raw beef. Corrected.
- A container of cooked chicken was found with no date marking. Person in charge (PIC) stated that the chicken was made on 3/13. When keeping time/temperature control for safety foods (TCS) that are ready to eat (RTE) for longer than 24 hours, a date that the food item was opened/prepared, or a discard date not to exceed 7 days from the date that the food was opened/prepared shall be labeled on the food/container. PIC date labeled the container of chicken with 3/13. Corrected.
- Several containers of frozen/refrigerated foods were found stored with no cover. Foods shall be protected from sources of contamination such as dust, dirt, splash, drips etc. by covering foods or providing other means of protection. PIC covered containers of food during the inspection - Corrected.

- Total violations: 6
- Both bar hand washing sinks do not have hand soap available for employees to properly wash their hands. All hand sinks shall have hand soap available for proper hand washing.
- Raw shell eggs observed stored improperly over ready to eat foods (vegetables and creme brulee cups).
- Potato wedges observed cold holding at improper temperatures (53F) at the prep cooler on the line (wedges over stacked in pan). TCS (time temperature control for safety) foods shall cold hold at 41F or less.
- 2 chemical spray bottles observed without labels in the kitchen. All spray bottles need to be labeled with their contents.
- Observed several time and temperature control for safety foods (TCS) not properly dated for disposition. Foods not date labeled include cooked chicken wings, portioned rice, butter chicken sauce.
- In use utensils observed improperly stored in a 6 pan of room temperature water.

- A container of Chimichangas was found with no date marking in the low boy cooler on the line, a container of Beef Brisket with no current date mark (1/25/23 - Frozen) was found in the reach-in cooler. Time/Temperature control for safety foods (TCS) that are ready to eat (RTE) that are held over 24 hours must be labeled with the date that the food was opened/prepared or with a discard date not to exceed 7 days from the date that the item was opened/prepared. Person in charge (PIC) date labeled the chimichangas and relabeled the beef brisket. Note: When cooking and freezing foods the same day to use at a later date, the date that the food is made/frozen counts as day one. A date label once the food is removed from freezing to thaw will resume the 7 day time period ex: cooking/cooling/freezing = 2 days, once pulled to thaw/defrost the food must be used/consumed/sold within 5 days and no more. Corrected.
- No test strips to measure the Quat Sanitizer at the three basin sink are available for use - none on premise. Testing strips or other device that measures & monitors the concentration of sanitizers must be provided. PIC is ordering more, EHS gave test strips to use in the interim. Corrected.
- Several lights over the cooking line & prep line are not covered or shielded and exposed. Lights shall be covered/shielded so that they are not exposed and if broken, are contained so they do not fall on employees or into foods. Cover/shield these lights ASAP.

- No hand towels were observed at the hand sink in the kitchen. Hand sinks must have proper hand drying provisions. Person in charge provided paper towels for the hand sink during the inspection. Corrected.
- The low temperature dish machine is not dispensing sanitizer - 0ppm when tested. Food contact surfaces shall be sanitized before use, after cleaning. Person in charge (PIC) will use the three basin sink to wash/rinse/sanitize until the dish machine is repaired. PIC placed a service call to repair the dish machine during the inspection. Service is scheduled for later today 3/16. Corrected.
- Spring rolls and summer rolls were observed with no date marking. When keeping time/temperature control for safety foods (TCS) that are ready to eat (RTE) longer than 24 hours, a date that the food item was opened/prepared or a date not to exceed 7 days from the date that the food items were opened/prepared shall be labeled on the food item or its container. PIC date labeled both containers of summer rolls and spring rolls. Corrected.
- Time as the public health control (TPHC) is used for reconstituted noodles and shredded cabbage during lunch and dinner rush. EHS observed no written time stamps or log to monitor the time that these food items are kept at room temperature and when to discard them. When using TPHC a written log/label/process must be verifiable to ascertain when TCS/RTE foods are taken from temperature control and when they shall be discarded. PIC provided time stamps on the containers of the food items used for the lunch rush. Corrected.
- EHS observed newspaper lining the bottom of a storage container for Spring Rolls. Newspaper is not an acceptable food contact surface. Food contact surfaces must be smooth and cleanable and able to be sanitized. They must not be porous, absorbent or lend to chemical contamination through seeped ink or other components. Discontinue use of newspapers immediately.

- Ready to eat food items found were not heated to a sufficient temperature for hot holding in that pulled chicken, pulled beef registered at 152°F for reheating temperature.
- Time/temperature control for safety (TCS) foods must be held at 41°F or below in that horchata (drink with milk) registered between 62°F in the self service drink station. As per the PIC said drink was placed in said location for an hour prior.
- Facility does not have test strip to measure the concentration of the sanitizer.

- Observed multiple containers of cut lettuce in metal containers covered and stacked on top of each other in the walk in cooler. When lettuce is cooling the lid should be loosely covering or uncovered.
- Observed white fish and salmon thawing on rack in the kitchen. Thaw time/temperature control for safety foods by one of the following methods: 1. Under refrigeration that maintains food temperature at 41 F or less, 2. Completely submerged under running water that is a) at a temperature of 70 F or below; b) with sufficient water velocity to agitate and float off loose particles in an overflow; c) for a period of time that does not allow thawed portions of ready-to-eat to exceed 41 F; d) for a period of time that does not allow portions of raw animal foods requiring cooking to exceed 41 F for more than four hours, or 3. As part of the cooking process. Person in charge moved fish to the walk in cooler.

- Chicken wings in the tall reach in cooler observed at 46 degrees. Manager stated that they were cooked yesterday afternoon. Cool time/temperature control for safety (TCS) food(s) from 135F to 41F within a total of 6 hours and from 135F to 70F within 2 hours.
- There is an opening between the floor and the back door, outer openings should have a screen mesh to prevent the insects from entering the facility.
- An irreversible registering temperature indicator was not provided for the hot water mechanical warewashing operation. Provide the thermometer to make sure kitchen utensils are being sanitized.

- Management must be aware and inform all employees of their responsibility in reporting symptoms or diagnosis of diseases transmissible through food. Manager was unable to provide a signed copies of employee health reporting agreement.
- The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
- Three compartment sink observed with only spraying hose.

- Time as the public health control (TPHC) is used for cut lettuce & Cut tomatoes that are stored at room temperature on the line. EHS observed that the time & date stamps on these items was incorrect, showing 1-3 am @ 3/17 therefore the written log/monitoring system is not operating correctly and time is not being properly managed. Manager was notified and was able to correct the timing error within the machine. New time/date stamps were applied to food items that use TPHC. Corrected.

- Observed light showing through the back door, which could allow for possible pest entry. Observed one fly and one insect in back of house. A wet rug is shoved under door absorbing water.

- Employee's uncovered coffee cup stored on the kitchen counter by the prep cooler. Employees drinks should be covered where contamination of exposed food or clean equipment may occur.
- Hand sink in the back doesn't have a working soap dispenser. Provide soap by the hand sink to allow employees wash their hands.
- Time/temperature control for safety (TCS) food should be date marked if stored for more than 24 hours, and discarded after 7 days.
- Working containers of soap/water are not properly labeled with the common name. Properly label working containers of toxic items with the common name.

- Establishment is unable to provide procedures for employees to follow when responding to vomiting or diarrheal events.
- Ready to eat TCS foods held over 24 hours must be date marked with either made on date or date the food item must be consumed not to exceed 7 days. Date marking not provided to several containers of fried chicken in the walk in refrigerator. As per the Person in Charge (PIC) said food was prepared two days prior.
- Sanitizer test strips not provided for the chemical dishwashers and three compartment sink at the kitchen and at the bar.

- Total violations: 7
- Person in charge advised yogurt was not made in the facility and was purchased. Upon further investigation, an employee advised they made the yogurt in house. This item was placed in an old yogurt container and was not honestly presented.
- Observed raw chicken stored over cooked rice in cooler. Prevent potential cross contamination by not storing raw animal foods over ready to eat foods.
- Chickpea (79), chicken curry (67), chicken jalfrezi (81), rice (90) These items were finished cooking around 8-830am this morning. They were placed in small pans on ice to facilitate cooling then put into large rubber bins and left on prep table. Person in charge advised that food was left here because they may need it throughout the afternoon to refill the buffet. These items noted as improperly cooled to prevent the growth of harmful bacteria. Cool time/temperature control for safety (TCS) food(s) from 135F to 41F within a total of 6 hours and from 135F to 70F within 2 hours. Went over the possibility of using time as the control for lunch buffet items but it was decided temperature would be used as the control so facility did not have to discard unused food.
- Observed chickpea curry, spinach and chicken curry (111, 108, 120F) hot holding at improper temperature (F). Time/temperature control for safety (TCS) foods must be hot held at 135F or above to prevent growth of harmful bacteria.
- Observed time/temperature control for safety foods (TCS) not properly dated for disposition. Foods not labeled include: chickpea meal, chickpea potato appetizer, lentils, hard boiled eggs, sauces. Ready to eat TCS foods held over 24 hours must be date marked with either made on date or date the food item must be consumed not to exceed 7 days.
- Spray bottle at dishmachine does not have label to identify contents.
- Person in charge advised yogurt was purchased from Costco. The containers of yogurt in facility did not appear to be store bought. Employee advised he made the yogurt in house. There is no variance granted for the making of yogurt for sale or service in a food establishment. Facility ran out of yogurt yesterday and has made their own. Making ones own yogurt requires and approved variance to be applied for. Discontinue practice of making yogurt.

- Total violations: 4
- Handwashing station observed being used for other purposes than to only wash hands in that the handwashing station at the front bar observed with strainer and used dirty utensil being placed in them. A handwashing sink may not be used for purposes other than hand washing.
- Handwashing facility at the bar observed without soap. Hand soap must be provided at all hand washing sinks to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.
- High temperature dishwasher not reaching the required temperature to sanitize the dishes in that during inspection the maximum temperature recording thermometer was observed reading below 160°F for sanitization. The temperature records also indicated that sanitization temperature had been below 160°F in morning.
- During the inspection several containers of milk in the self service coffee area held using time control when no written procedures for the use of time as a public health control with time/temperature control for safety (TCS) foods are available.

- The hand sink located to the left of the three basin sink does not have available hand cleanser. All Hand sinks that are utilized by food employees must have accessible hand cleanser. It appears that the battery to the soap dispenser is dead, therefor the dispenser is unable to dispense soap. Provide a hand cleanser at all times.
- Sandwiches that are premade and stored in the sandwich cooler on the line do not have date marking on them. Time/temperature control for safety foods (TCS) that are ready to eat (RTE) must be labeled with the date that they are opened/prepared or a discard date not to exceed 7 days from the date that the food item was opened/prepared. Date mark sandwiches.
- Pork that had been fried was observed cooling in a covered container at room temperature. When cooling foods, do not cover until the item has cooled. Person in charge moved the container to the walk-in cooler and removed the cover so that the item can properly cool. Corrected.
- There are no test strips to monitor the concentration of sanitizer at the three basin sink. Test strips or other testing device must be provided to monitor sanitizing concentrations. Provide QUAT testing strips for the three basin sink,

- There is dried food residue on slicer after it has been washed/rinsed/sanitized. Food contact surfaces shall be properly cleaned and sanitized after using.

- Food employee observed eating on rear food prep table in rear food prep area. Food employees can eat in designated non food prep/storage area.
- Soap was not provided at the hand washing facility in the front kitchen.
- The prepared ready-to-eat (RTE) food held more than 7 days in the refrigerator in that prepared tomato soups (tomato soup) observed dated to be held for 10 days. Ensure all prepared RTE time/temperature control for safety (TCS) food is date marked if the food is to be held for more than 24 hours. The food shall be served, sold or discarded within 7 calendar days with the day of preparation counted as day 1. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.
- Improper thawing in that raw chicken observed placed in the prep sink at room temperature.
- Few live flies and fruit flies observed in kitchen dishwashing area.

- Observed 3 of the hand washing sinks to be used to store items, including: Steel Wool in all 3, Scoops, Bowls, Strainers.
- Observed holes/openings/white light under door in back of house and screen door is not sealed either/has holes in screen. Could allow for possible entry of pests.
- Observed several bugs on glue trap.
- Observed Live Roach in the Dishwashing Machine.
- Observed in use knives in-between prep table.
- Observed washed pots and pans air drying on rack upside down with water pooled in various dips and depressions of handles/ other parts of the pots and pans preventing proper air drying.
- Observed Dishwashing Machine ES2000HT to be functioning with an atmospheric temperature of 187 (verified by inspector - Temp Rite Strip, Gauge), however there is no means to verify.
- Observed that Ice Machine’s Guard inside machine has black residue.
- Observed several employee items all throughout back of house, including: keys, jackets, coats, and backpacks.

- The facility was unable to produce a written policy/procedure for the clean-up of vomit or diarrheal incidents. A written policy or procedure for employees to follow to properly and effectively clean up any/all vomit or diarrheal events must be provided. EHS printed the NOROVIRUS worksheet in both Spanish & English and gave to the person in charge (PIC). EHS also included these as attachment to the food safety officer that oversees this facility. Corrected.
