Ashburn Health Inspections: Live Roaches In Restaurant
The Loudoun County Health Department recorded violations at several facilities around the Ashburn area during recent inspections.
ASHBURN, VA — Several facilities in the Ashburn area recorded violations during health department inspections between Aug. 23 and Aug. 29.
COVID-19 restrictions around the state have relaxed, so health inspectors conduct most restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

- Total violations: 2
- Time/temperature control for safety (TCS) foods must be held at 41°F or below in that chicken tenders registered at 54°F in a holding unit at room temperature in the kitchen. As per the PIC said chicken were placed in said unit 30 minutes prior to hold for frying during the rush time.
- The door of the low boy refrigerator holding TCS food items observed not closing completely as the gasket of said unit observed torn and not completely sealed.
- Aloft Hotel RE: Fuel, WXYZ Bar, 22390 Flagstaff Plaza Ashburn, VA 20148

- Observed that paper towels not provided at the kitchen handwashing sink.
- Cooked eggs, ham, cheese, sausages registered at 60°F in the walk in refrigerator in kitchen.
- Improper date marking provided to cooked oatmeal and in the walk in refrigerator. Said food observed dated to be held for 9 days. Prepared TCS food items can be held for 7 days from the day they are prepared.

- The back hand washing sink near the dish machine was blocked by a large trash cans and surrounding dough proofing racks. All hand sinks shall be easily accessible at all times.
- Bags of wings cold holding at improper temperatures (46F) at the beverage air cooler next to the fryers.

- No paper towels provided at the hand sink in the back prep kitchen. Hand drying provisions shall be provided at all hand sinks at all times to facilitate proper hand drying after hand washing.
- Meatballs observed hot holding at improper temperatures (98F) at the steam table. The steam table was very low on water.
- The insect control device in the kitchen is visibly full of dead insects. Said device is mounted near the ceiling and near/above the prep table. This may result in unintended contamination of open food being prepped on the prep table. Facility is undergoing a major remodel starting on 9/3/2023 EHS discussed the relocation of said device with the person in charge.

- Handwashing station observed being used for other purposes than to only wash hands in that the handwashing station at the front bar observed being used as dump sink with strainer placed in it.
- Raw steak observed placed over cut onions and peppers in the low boy refrigeration unit in the kitchen,
- Bar dish machine in use has no chlorine output to sanitize.
- Raw salmon registered at 50°F in low boy drawer refrigerator. Said unit holding other TCS food items at adequate temperature.

- There is a gap between the back door and the floor. Cover the gap to prevent entrance of rodent and insects.
- Inside of the ice machine observed with accumulation of black residues.

- Observed the 3 -compartment sink in bar the quaternary at 0ppm. The quaternary should test between 200-400ppm.
- Observed ice cream scoop in stagnate water. The ice cream scoop should be under running water or in water that is 135F or above.
- Observed the accumulation of soil residue from splashing of juice that has accumulated on the top plastic part where juice spouts pour juice. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

- No water available at the handwashing sink located in the dish machine area. The water lines had been turned off due to hole in the plumbing. All handwash sinks are required to have adequate warm and cold water for proper handwashing. The service station handwash sink will be used temporarily until the repair is complete.
- Shellfish tags are kept chronologically for 90 days; however, the months of May and June are missing the date the last shellstock were sold/served as part of required record keeping.
- Farm raised salmon is served cooked to order. The menu is missing the corresponding asterisk to the consumer advisory. The consumer advisory informs the consumer of the increased risk of consuming undercooked fish.
- A stainless-steel pot of brown gravy was found tightly covered with plastic wrap improperly cooling inside the reach in refrigerator with an internal temperature of 68F for approximately 40 minutes. Gravy is required to be cooled without a lid until it reaches an internal temperature of 41F or below to prevent the growth of harmful bacteria.

- The establishment was unable to provide documentation that employees have been trained in the signs and symptoms of food borne illness and how to report them. Verifiable means that employees have been trained in the signs and symptoms of food borne illness and how to properly report them must be available for review when requested. EHS printed form 1B in Spanish and both employees onsite read and signed the document. Have ALL employees sign and keep within the facility so they may be reviewed when requested.
- There is no written procedure or policy that directs the proper clean-up of vomit or diarrheal events. A written policy or procedure to effectively clean and sanitize vomit or diarrheal incidents must be available to employees. EHS printed the NOROVIRUS Clean-up worksheet. Keep this in an area easily accessible to employees.
- The hand sink located to the left of the three basin sink in was found with multiple items stored within its basin. Hand sinks shall be clear of any obstructions and easily accessible at all times to employees. Items were removed from the hand sink so that it is not blocked and easily accessible. Do not use this hand sink for anything other than washing hands.
- 3 containers with water were found holding ice cream scoops. In-use utensils must be held between uses on a clean, dry surface, in water that is 135F or more or in continuously running water with enough velocity to wash away food particles. Containers/scoops were removed and replaced with clean dry scoops/containers.
- Facility has no testing strips or other device to measure the concentration of sanitizer (chlorine). Testing strips or other testing device must be supplied to test and monitor the concentration of sanitizer used. EHS provided testing strips until the facility is able to procure their own. Keep testing strips on hand within the facility at all times.

- A container of red snapper in reduced oxygen packaging (ROP) was found thawing on the line without the packaging cut. When thawing fish in ROP, the vacuum seal must be broken BEFORE the thawing process to allow oxygen into the packaging to eliminate the growth of harmful anaerobic bacteria. Packages were cut upon to introduce oxygen.

- Chemical spray bottle observed without the proper labeling. All working containers of toxic materials should have a label with the common name of the material in it.
- Improper methods used to thaw gyron cone. Gyro cone observed stored on the kitchen counter.
- Multiple live roaches observed in the facility. Manager stated that they have pest inspection once a week. It was requested to contact them right away and schedule an inspection.
- Falafel scoop stored in the water.

- Raw salmon stored beside the vegetables in the cooler. Raw animal origin food should be stored below ready to eat food.
- Frozen salmon observed thawing in the vacuum packaging. Remove the packaging to prevent any food contamination.

When keeping time/temperature control for safety (TCS) foods that are ready to eat (RTE) over 24 hours, a date marking the day that the food item was prepared/opened or a discard date not to exceed 7 days from the date that the food item was prepared/opened shall be labeled on the food/container. Date label any/all TCS/RTE foods that are kept over 24 hours.
- The salt, sugar and MSG on the line are not labeled and therefore unable to be distinguished. When taking foods/ingredients from their original packaging that are not unmistakably recognizable, a label with their common name must be on the working container. Label the salt, sugar and MSG containers on the line.
