Ashburn Health Inspections: Longhorn Steakhouse, Manhattan Pizzeria
The Loudoun County Health Department recorded violations at several facilities around Ashburn, Dulles, and Sterling in recent inspections.
ASHBURN, VA — Several facilities in the Ashburn area recorded violations during health department inspections between Nov. 16 and Nov. 22.
Most COVID-19 restrictions around the state have relaxed, so health inspectors are returning to conduct some restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

- Observed the hand sink in the back area to be leaky with a fluctuating temperature and the hand sink at the bar area to have a low flow.

- Black residue was observed on the inside portion of the ice bin of the ice machine.
- A large bin of fried rice was found cooling at 127F with a cover in the walk-in cooler.
- A container of raw onion was observed stored directly on the floor in the back portion of the kitchen, boxes of raw chicken observed stored on the floor of the walk-in cooler.

- Observed lettuce and tomatoes in kitchen that is using time as a public control without the appropriate documentation.
- Observed cooked corn and cooked shrimp without date marking.
- Observed cooked lobster 58F in ice bath all cold held food should be 41F or below.
- Observed mashed potatoes 120F on steam table hot held foods should be 135F or above.
- Observed a cleaned knife ready for use with food debris on blade.
- In women's restroom there was no soap.

- Verifiable means of an employee health reporting policy were unable to be produced during the inspection.

- Hand towel dispenser at the handwashing sink behind the bar is empty - no method to properly dry hands after washing.
- PIC stated that salmon that is served is cooked completely - menu shows a consumer advisory for salmon. If menu reflects a consumer advisory, proper parasite destruction paperwork must be on file, or consumer advisory removed from these items on the menu.
- Knives found stored in the wall mounted knife block near the dishwashing area were found with food debris on them.
- Personal items from employees (Purse, drinks, containers) was observed stored with take out containers and in dry storage.
- This facility does not have test strips to monitor the sanitizing agents for the low temperature dish machine (Chlorine) or the three basin sink behind the bar (Quat).
- The hand sink located at the end of the cook line was observed with a moderate leak coming from the draining pipe, and was also observed very slow to drain.

- Two hand sinks in the back kitchen do not have hand washing reminder signs.
- The meat slicer in the back kitchen observed with a build up of dried on food debris below the blade and in hard to reach areas.
- Pan of gyro meat observed sitting out at room temperature on the cook line. Gyro meat temperatures range from 99F (bottom of pan) to 136F (top of pan) Upon discussion with the PIC it was determined that the goal for this practice was to "cool" said gryo meat down before re- cooking it to order on the flat top griddle. The PIC said that the gyro meat was sitting at room temperature for 2-3 hours.
- Hoagie sub deli meat date marked 11/8/2022. Sliced ham dated marked 11/9/2022. Sliced deli meat in walk in cooler date marked 11/10. Said TCS (time/temperature control for safety) food was held longer than the allowable 7 days in the refrigeration unit.
- Facility now offers packaged baked goods (rice krispy treats, brownies, baklava, cheesecake etc.) in a grab and go manner. THe packaged desserts are not labeled with their ingredients.
- Wiping cloths (some wet) used to cover food in pans (gyro meat).
- Knives stored in visibly dirty knife holders at both the cook line and the prep table near the mixer. 3rd pan (plastic food pan) without a handle directly stored in marinara sauce for dispensing.

- 2 boxes of raw bacon observed stored over ready to eat (RTE) cheese slices, cut tomatoes, cut lettuce.

- Handwashing facility at the bar observed leaking and out of service. As per the PIC said hand sink has been out of service for 3 days.
- Handwashing facilities in front pizza prep area and in bar area observed without paper towels.
- Chemical dishwasher at the bar area observed not sanitizing.
- Improper date marking observed in that Temperature Control for Safety (TCS) food items in the low boy refrigerator observed improperly dated.
- The cutting boards are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.

- Observed pumpkin cheesecake and raw hamburger patties stored uncovered in the WIC.

- No paper towels provided at grill area hand sink.
- Observed raw bacon stored above RTE milk and cheese in the walk-in cooler and raw chicken stored above RTE pineapple slices and veggie patties in the line prep cooler.
- The following expired foods observed stored in the walk-in cooler: jambalaya with an expiration date of 11/10/22 and chipotle sauce with an expiration of 11/5/22.
- Observed 2 cans of Black Flag pesticide spray stored in mop area.
- Observed ice cream scoop stored in stagnant ambient water.
- Observed excessive accumulation of ice and ice particles in the walk-in freezer. Doors of both the walk-in freezer and walk-in cooler do not close completely on their own. Ambient air temperature of WIF noted at 21F and 40F in the WIC. Foods in WIF observed frozen and all temperatures in WIC at or below 41F.

- The sink at the front coffee counter has a sign reminding employees to wash their hands however it is not equipped with hand soap and paper towels. There is also a sink at the bar with a hand washing sign and no soap and paper towels.
- Food employee observed washing a pan by hand at the pre wash station near the dish machine. The food employee did not sanitize the pan after washing it. When tested, the low temperature dish machine registered at 0ppm sanitize. All wares being washed through the dish machine are not getting sanitized.

- Facility has gone back to self serve creamers/milk in caraffs. Said dairy products are on a 4 hour time control however the caraffs were not marked with a time stamp indicating when the dairy had come out of temperature control.

- At time of inspection person in charge could not provide verifiable documentation that food employees have been trained on the signs, symptoms, and exposures of foodborne illnesses.
- The hand sink by the 3 compartment sink had to soap.
- The hand sink in the bar and bakery had no paper towels.
- Observed bakery items that were optioned from an unapproved food source (a kitchen that is not inspection).
- In walk in cooler raw shell eggs stored next to ready to eat leafy greens.
- Observed wet wiping cloths in a variety of locations not in a sanitizing solution.
- Observed utensils used for rice in stagnant water at a temperature of 68F.

- Hot water provided does not meet minimum requirements for hand sink and 3 compartment sink. Observed hot water at hand sinks at 96F, 97F, and 97F at 3 compartment sink.
- The following areas of the physical facilities observed unclean: ceiling light shields, floor beneath grill, ice machine exterior filter cover.
- Observed many pieces of unused equipment and equipment in disrepair stored throughout kitchen.
- Observed ceiling light bulbs unshielded.
- Lighting throughout kitchen is insufficient in meeting minimum requirement. Facility is experiencing electrical issue with ceiling lights; temporary constructions lights are in place.

- 2 employee drinks (one with no lid/straw, one with lid and straw) were found stored on shelving designated for the storage of clean plates & utensils.
- Sheets of raw bacon observed stored over cooked, ready to eat (RTE) pasta in the walk-in cooler.
- A gallon of whole milk was found in the walk-in cooler at 53F, outside of proper cold holding temperature requirements.

- Preparation of raw food conducted in a manner that may cause cross contamination of to ready-to-eat food (RTE) in that food employee observed prepping raw ground beef on top of cutting board on sandwich prep cooler with the open containers of cut tomatoes and lettuce next to them.
- Date marking not provided to some of the food items in the walk in refrigerator such as marinara sauce. As per the PIC said food item was prepared a day prior.
