Ashburn Health Inspections: Los Toltecos Bar & Grill, The Pit Stop

The Loudoun Health Department found violations at several restaurants in the Ashburn area during recent inspections.

ASHBURN, VA — Several facilities around Ashburn recorded violations during health department inspections from Sept. 7 to Sept. 13.

Most COVID-19 restrictions around the state have relaxed, so health inspectors are returning to conduct some restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection

      • Observed hand sink in the new prep area to not have soap accessible.
      • Observed handwashing sink in the new prep area to not have paper towels or an approved method of hand drying within arms reach.
      • Observed several fruit flies in the back near dry storage where plantains are stored.
      • Observed in use tongs stored on oven handle in kitchen.

      • Chemical dishwasher in kitchen not sanitizing adequately. As per the person in charge (PIC) the technician came to repair the broken pipe however, the dishwasher is not sanitizing.

      • Observed Bean Sprouts stored on ice at 72F at line and 4 large bags with a temp of 48F in the Walk in.
      • Observed In Use Tongs stored on oven handle, Ice cup inside of ice in ice bin, and 2 in use knives stored under toaster oven.

      • Person in charge could not located employee health policy.

    • Total violations: 5
      • Verifiable means that employees have been trained in the signs and symptoms of foodborne illness are not available for review.
      • The facility does not have a written policy on how to properly clean-up and disinfect vomit or diarrheal events.
      • There are no paper towels in the paper towel dispenser in the kitchen at the hand sink to properly dry hands.
      • This facility is using time as the public health control (TPHC) for Cream cheese, milk and hard boiled eggs but does not have written verification of this process to monitor times that food is not under temperature control.
      • Two boxes of waffle mix are being stored directly on the floor of the kitchen.

      • Observed: Walk in refrigeration is not maintaining cold holding temperatures at 41F or below.

      • There is no signage at the hand washing sink to the left of the three basin sink reminding employees to wash their hands.
      • EHS observed no soap at the hand washing sink to the left of the 3 basin sink/prep area.
      • Raw chicken was found stored over raw beef and Pork in the walk-in cooler. Raw shell eggs were observed stored above ground beef in the walk-in cooler.
      • Date marking of time/temperature control for safety (TCS) and ready to eat foods (RTE) is not being conducted.
      • Employee was observed with no hair restraint.

      • Mold on ice deflector.
      • Sausage/meats held over 24hrs with no date labeled.
      • Consumer advisory on menu does not have foods it pertains to marked.

      • A sign or poster that notifies food employees to wash their hands is not provided and at the handwashing sink in bar area.
      • Few fruit flies observed in bar area.
      • Sanitizer test strips observed discolored.
      • Inner panel of ice machine observed with accumulation of pink mildew.

      • EHS observed pork buns in packaging in the customer access cooler at the front of the store. After questioning it was determined that the pork buns are not prepared by this facility, and are purchased from an unapproved food source.
      • Time/Temperature control for safety foods were found cold holding at improper temperatures in the low boy cooler directly to the right upon entering the kitchen space. Sliced Papaya (53F), Pate (54F), Cut lettuce (54F), Meat wonton (53F), Waffle mix (54F), Crepe Mix (53F).
      • A container of HOTSHOT pesticide was found hanging on a cabinet door at the prep area.
      • Several containers of utensils stored in stagnant water were found throughout the kitchen.

      • Eggs-raw eggs are holding over Ready to eat (RTE) foods in cooler.
      • Hard boiled eggs made and are cooling after 3-4hrs sitting out on prep table. Current temperature is 82F.
      • Tandoori chicken is holding at 100F currently in hot hold unit which is not hot enough (112F) to hold heated items. Must hot hold foods at 135F or above.
      • Spray bottles of toxins in kitchen are not labeled.

    • Total violations: 5
      • Observed inside of microwave to be unclean.
      • Observed Cooked Chicken (prepared yesterday) in a hot holding unit with a temperature of 80F heating up.
      • Observed previously prepared chicken stored in unit. MGMT indicated that food is held for 3-4 days once prepared.
      • Observed several unlabeled bottles containing bleach.
      • Observed walls behind 3 VAT and prep table to be unclean.

      • Soda Nozzles contain mold like substance when checked with alcohol swab.
      • Trash out back/supplies.

      • Observed the following line items above 41F: Gyro @ 43F, Roast Beef @ 44F and Turkey @ 45F.
      • Hobart AM16SCB-16 was not operational at preopening. After 6 weeks of operation AM16SCB-16 is still not operational.
      • Observed Mops drying pressed against wall.

    • Total violations: 2
      • Raw shrimp observed stored over ready-to-eat lettuce in the refrigeration unit.
      • Facility observed using time as a public health control for sushi rice without proper written procedure.

    • Total violations: 7
      • Handwashing facility in the rear kitchen observed blocked with plastic container and fork placed in it.
      • Low chemical dishwasher in bar area not sanitizing after wash and rinse cycle.
      • Large container of cooked beef registered at 45 degrees F in center on shelf in the walk in refrigerator. Said food was cooked a day prior and had been cooling in the walk in refrigerator for more than 12 hours.
      • Several live fruit flies observed near the three compartment sink in the kitchen. Dirty wiping cloths observed on floor in said area.
      • Dirty wiping cloths observed on floor near three compartment sink.
      • Maximum temperature recording thermolabel not provided to measure the sanitizing temperature for the high temperature dishwasher.
      • Ice machine observed with mildew build up on inner panel. Said ice used for consumption.

      • No CFM present, 2 reachable CFMs have expired certifications.
      • Observed no paper towels at front hand sink.
      • Observed meatballs at line hot holding at 118F.
      • Observed Quat to be over 500 ppm and out of range.

      • Observed Ambient Air Thermometer in Small Refrigerator to be broken.
      • Observed light showing through the back door, which could allow for possible pest entry.
