Ashburn Health Inspections: Moldy Ice Bin, Live Roaches, Dead Flies

The Loudoun County Health Department recorded violations at facilities around Ashburn, Dulles, and Sterling during recent inspections.

ASHBURN, VA — Several facilities in the Ashburn area recorded violations during health department inspections between Dec. 7 and Dec. 13.

Most COVID-19 restrictions around the state have relaxed, so health inspectors are returning to conduct some restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

    • Date inspected: Dec. 7
    • Total violations: 2
      • No paper towels at hand sink.
      • Small prep cooler had been accidentally unplugged this morning. Items in cooler were 59-62F. As food is not kept in this cooler overnight and had been placed in equipment recently, cooler was turned on and food may be kept.

    • Total violations: 2
      • Half gallon of open buttermilk in walkin cooler had date of 11/29/22, had a foul smell and the product had separated. This item was 8 days past its date.
      • Foods with discard date past today's date in walkin cooler. Cocktail sauce 12/3 Spicy remoulade 12/3 Chipotle lime 12/5 Chicken marinade 12/3.

    • Total violations: 7
      • There is no person in charge with a valid Food Protection Manager Certification from an accredited agency recognized by the Conference for Food Protection.
      • Signed copies of food employee reporting agreement (Form 1B) not provided at the facility.
      • Hand washing sink next to the three compartment sink in kitchen observed without paper towel.
      • Temperature control for safety (TCS) food items not held at 41°F or below in that cream cheese registered at 54°F in front customer self service area.
      • No written procedures exist for the use of time only as a public health control for cut cantaloupes and milk in thermos in self service area. As per the person in charge(PIC) cut cantaloupes and milk in thermos are using time as a public health control.
      • Working sanitizer test strip for quaternary ammonia not provided during inspection.
      • Maximum temperature recording thermometer or thermolabel not provided for high temperature dishwasher.

      • Handwashing facility in the kitchen observed without soap.
      • The food is not protected during storage in that one small reach in refrigerator holding open containers of juice for cocktail and a dehydrator for fruits to garnish the drinks were observed outside the liquor storage room in area that is accessible to the public.

      • There is no certified food protection manager for this mobile food unit. The restaurant associated (commissary) with this mobile food unit does have a CFPM but they do not work on the truck.

      • The facility did not have verifiable means of a procedure or policy in place to properly clean and disinfect vomit or diarrheal incidents. EHS discussed proper clean-up and gave the person in charge (PIC) a copy of Norovirus cleanup worksheet in English and Spanish.
      • No paper towels or other means to dry hands after washing them was observed at the hand sink on the cook line. PIC replaced paper towels.
      • Two live roaches were observed during the inspection. EHS requested pest control measures that the facility was taking to eliminate pests. EHS discussed with PIC need to eliminate harborage conditions.
      • A personal cell phone was found stored on the counter at the breading station. PIC removed the phone to the designated employee area.
      • Two rice scoops were observed stored in between uses in stagnant water in the dipping well on the line. The dipping well needs to be turned on and at a velocity strong enough to wash away food particles. PIC turned on the dipping well.

      • Food employees observed drinking tea from open cups/container in kitchen.
      • Beef egg rolls registered at 60°F next to the fryer at room temperature. Said egg rolls were taken from the walk in refrigerator and placed them in said location for reheating and serving.
      • Date marking not provided to beef egg rolls and brown rice prepared three days prior held in boxes in walk in refrigerator. Said food is held up to 7 days as per the PIC.
      • Dirty linen used to cover the spring rolls in the freezer in the kitchen.
      • Ventilation hood in kitchen observed with heavy accumulation of grease.

      • Observed microwave to have accumulation of aged food debris.
      • Observed in use tongs stored on oven handle pressing on oven door.

      • Marinara sauce prepared on 12/11/2022 noted as improperly cooled in the walk in cooler. Sauce measured at 52F.
      • EHS informed by food employee that bulk bags of frozen sauce are thawed at room temperature in the prep sink overnight. Sauces are not thawed under running water. This is an improper thawing method.

      • A step ladder was observed stored directly above food prep sink. The ladder is dirty, and debris could fall off onto the food that is prepped in the sink.
      • Employee was observed washing and rinsing food trays and pans then putting cleaned dishes on storage rack without sanitizing.
      • A can of WD-40 was observed stored on shelf directly above slicer used to cut chicken. This could lead to a chemical contamination if can is knocked over on slicer.
      • When manage was asked to check the concentration of the chlorine sanitizer at dish machine, he could not find any testing strips to check the concentration.

    • Total violations: 11
      • The only signed employee health policy papers are for employees that no longer work in the establishment. The employees present today do not have a verifiable employee health policy on file.
      • Employee drink improperly stored uncovered on a shelf over a prep table on the cook line.
      • A hand washing sign at the hand sink by the walk in cooler reminding employees to wash their hands is missing.
      • No paper towels provided at the hand sink near the dish machine and also at the hand sink near the walk in cooler on the line. There is also no sign at the hand sink by the walk in cooler reminding employees to wash their hands.
      • A food employee was observed mixing raw chicken with a marinade. The food employee was preparing this said raw chicken on a prep table that had ready to eat (RTE) chicken and a bag of romaine lettuce (also Ready to eat) in close proximity to the raw chicken. EHS observed the marinade from the raw chicken splash the fully cooked chicken and the bag of romaine lettuce. The fully cooked RTE foods were not protected from cross contamination with the raw chicken.
      • Raw chicken improperly stored over raw shell eggs in the walk in cooler.
      • Biryani rice noted as improperly reheated for hot holding. The rice was pulled from the walk in cooler at 11am and placed in the rice cooker to reheat. WHen checked by the EHS at 1:37 pm the rice registered varying temperatures from 55F- 100F.
      • Several ready to eat TCS foods are not date marked for disposition. Dates missing from the following foods: - Fried vegetable fritters in low boy cooler (3 containers) - Sauces in walk in cooler - Rice pudding in the glass door cooler.
      • Multiple spray bottles of chemicals stored on prep tables in the kitchen and next to single service items (to go containers)
      • Multiple containers of food improperly stored directly on the floor of the walk in cooler.
      • The facility does not have sanitizer test strips to test the sanitizer concentration of the low temperature dish machine. Employees do not have a way to ensure it is sanitizing at the correct concentration.
  • Hunan Village, 42920 Piccadilly Plaza #131 Ashburn, VA 20147
    • Date inspected: Dec. 13
    • Total violations: 12
      • The person in charge (PIC) is not monitoring employee food safety practices or food safety procedures. PIC is not aware that ready to eat food is stored in raw chicken shipping box, test strips are not available and not used, dish machine sanitizer is not available, employees are eating in kitchen, and hand washing is not being conducted appropriately due to lack of supplies at all hand sinks. The CFM not on duty during inspection.
      • Employees observed eating food in kitchen near grill area. Facility does not have a designated area for employees to eat away from customer food and clean food contact equipment.
      • Food handler observed eating at kitchen prep cooler, scratching face and head, then proceeded to covering container of rice with plastic wrap without having first washed hands
      • No soap for washing hands provided at the following hand sinks: dish room area and customer service area.
      • No paper towels observed at all 3 kitchen hand sinks, food handlers are not provided means to dry hands after washing.
      • Many containers of washed and cut vegetables stored directly in cardboard shipping boxes.
      • Observed prepared egg roll filling, shredded cabbage and carrots, stored directly in cardboard box used to ship raw chicken.
      • No sanitizer available at dish machine. Chlorine sanitizer tested at 0ppm and does not meet minimum requirement of 50-100ppm.
      • Many containers and boxes of food stored directly on floor in kitchen, walk in cooler and walk in freezer.
      • There are no test strips available to measure the chlorine sanitizer solution concentration.
      • Floor in dry storage area by back door and mop sink observed unclean.
      • Ceiling light above dry storage area near back door is not lit, area observed dark and unclean.
