Ashburn Health Inspections: Moldy Tomato, Fly In Kitchen
The Loudoun County Health Department recorded violations at several facilities around the Ashburn area during recent inspections.
ASHBURN, VA — Several facilities in the Ashburn area recorded violations during health department inspections between May 3 and May 9.
Almost all COVID-19 restrictions around the state have relaxed, so health inspectors are returning to conduct some restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.
For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

- Total violations: 2
- A food employee was observed drinking from an open glass in an area other than designated break area while preparing/handling food or contacting clean food contact surfaces.
- No disposable towels were provided a the hand washing lavatory next to the walk in refrigerator in the kitchen.
- La Fe Bakery & Deli, 95 Sugarland Run Dr Sterling, VA 20164

- Total violations: 4
- No hand drying provisions were available at either hand sink in the front and back portions of the kitchen. Provisions to properly dry hands must be provided at all hand sinks that are utilized by food employees. Person in charge (PIC) had employee refill both hand towel dispensers. Corrected.
- Pork and cheese mix for papusas was observed cold holding at 67F, above the required 41F or less. Time/Temperature control for safety foods (TCS) that are ready to eat (RTE) must be maintained at 41F or less, unless time as a public health control is used. Papusa mix was taken from refrigeration at 10:00am. Mix was placed in the freezer to rapidly cool to 41F or less, and then will be moved to the refrigerator to maintain 41F or less. Corrected.
- Three containers of TCS RTE Foods (Black Beans, Cheese mix and pork mix) were observed with no date marking. When keeping TCS/RTE foods longer than 24 hours, a date that the food item was opened/prepared or a discard date not to exceed 7 days from the date that the food item was opened/prepared must be labeled on the container/food item. PIC had employee date mark all three containers during the inspection. Corrected.
- There is a leak/overflow issue with the main pipe leading into the grease trap. Leak appears to be from the elbow portion of the pipe, impeding proper flow of water and debris into the grease trap and leaking onto the floor. Discussion with the PIC - plumber recommends a pump to ensure that water and debris is properly flowing into the grease trap. Repair the plumbing so that it is not leaking on the floor and is draining properly.

- Large containers of paint stored with food in the dry storage area right next to a bag of rice. Separate toxic chemicals from food, food contact surfaces, equipment, utensils, and single service items.
- Clean knives and clean cutting boards stored on the back side of the three compartment sink on the sink that is used for washing dirty equipment. The knives and cutting boards are subject to splash from the cleaning of the dirty equipment.
- A ceiling tile is missing from the ceiling in the back kitchen which is exposing piping within the ceiling. This area is directly over a prep table.

- There are several items offered a packaged dry snack form to take home "Crispy Noodles, Plantain Chips, Candied Peanuts, Rice Crispy Khao Mao, and Sweet Crispy Noodles" which upon discussion with mgmt it was determined that they were made in unapproved home kitchens. Packages are not labled with ingredients or allergen statement.
- Observed raw chicken stored next to raw beef, raw shell eggs, and raw shrimp.
- Observed several prepared bags of noodles and pad thai. MGMT indicated that they were to sell over the next 2-3 day. Bags contain no date labeling.
- Observed light shinning through underneath the back door allowing for possible pest entry, observed 1 fly.
- Observed the insides of several cold holding units to be in need of a cleaning.

- Observed Tomato to contain mold like substance.
- Observed in use tongs for doughnuts stored in a basket with a few other items, tongs appear to not have been sanitized recently. Observed ice cream scoop stored in stagnant liquid.

- Observed raw chicken in prep cooler at improper cold holding temperature of 47F. Food held cold should be maintained at 41F or below.
- Observed a hole in the floor be back door. There should not be a hole to allow pests into the mobile unit. Doors and floors should be tightly sealed.
- Observed cutting board on prep cooler with heavy scratching and scoring. Making the cutting board not easily cleanable.

- The hand sink at the back of the kitchen was still obstructed by 5 gallon buckets, and pallets. Hand sinks utilized by employees must remain unobstructed at all times and be available for use to promote frequent hand washing. Person in charge (PIC) had employees remove the obstructions during the inspection. Corrected.
- The hummus and tomato/cucumber salad has been labeled with the name of the food and an expiration date but is still missing the main ingredients/allergens and the name of the establishment. Include this information on all labels used for these items.
- Foods were observed in bins stored directly on the floor. Food shall be protected from cross contamination by storing all foods at least 6" off the floor. PIC had foods placed on shelves and risers during the inspection. Corrected.

- Total violations: 4
- A container in the walk-in cooler was observed with raw chicken and raw beef stored together, separated only by thin plastic bagging. Food must be protected from cross-contamination by separating raw proteins from each other and arranging them from lowest internal cook temperature, to highest. Person in charge has employee separate the beef from the chicken and store in a separate container, above the raw chicken. Corrected.
- No date marking of foods was found during the inspection. When keeping time/temperature control for safety foods (TCS) that are ready to eat (RTE)for longer than 24 hours, a date that the food was opened/prepared or a discard date not to exceed 7 days from the date that the food was prepared/opened must be labeled on the food/container. Date mark all TCS RTE foods.
- Multiple packets of fish (Salmon/white fish) was observed thawing in reduced oxygen packaging (ROP) without the introduction of air. When thawing frozen fish in ROP, the vacuum seal must be broken so that oxygen may be introduced to eliminate the risk of harmful anaerobic bacteria reproducing. PIC cut open packages of fish to introduce oxygen. Corrected.
- A scoop used to dispense cooked rice was found stored in a container of stagnant water. When utensils that are in-use and are stored in between uses, they must be stored on a clean, dry surface, in water that is 135F or more, or in consistently running water that is 70F or less and with enough velocity to wash away food particles. PIC removed the utensil and replaced with a utensil in a clean dry container. Corrected.

- No supervisors or managers are certified food protection managers. At least one employee with supervisory and management responsibility and authority to direct and control food preparation and service shall be a certified food protection manager. This person shall show their proficiency of required information by passing a test that is part of an accredited program.
- Large container of chicken noted as improperly cooled to prevent the growth of harmful bacteria. Chicken was made at last night and has been cooling for more than 6 hours. Cool time/temperature control for safety (TCS) food(s) from 135F to 41F within a total of 6 hours and from 135F to 70F within 2 hours.
- Observed cooked chicken, potato balls in the reach in cooler cold holding at improper temperature (F). Time/temperature control for safety (TCS) foods must be held at 41F or below to prevent growth of harmful bacteria.
- Observed time/temperature control for safety foods (TCS) not properly dated for disposition. Foods not labeled include: cooked chicken, spinach, onion sauce, tomato sauce. Ready to eat TCS foods held over 24 hours must be date marked with either made on date or date the food item must be consumed not to exceed 7 days.

- Food employee observed handling (preparing/rolling) chicken Ceasar wraps with their bare hands. Ready to eat (RTE) foods can not be handled with bare hands.
- The one hand sink in the back prep sink is blocked with a rolling speed rack of bread. Food employees observed actively engaged in food preparation. All hand sinks shall remain easily accessible at all times for use by food employees.
- Sliced deli meats in the prep cooler observed cold holding at improper temperatures (48F). Deli meats observed stack high above the fill line of the pans in the prep cooler. Cold hold TCS (time temperature control for safety) foods at 41F or less.
