Ashburn Health Inspections: Roach At School, Gnats In Kitchen
The Loudoun County Health Department recorded violations at several facilities around the Ashburn area during recent inspections.
ASHBURN, VA — Several facilities in the Ashburn area recorded violations during health department inspections between Sept. 27 and Oct. 3.
COVID-19 restrictions around the state have relaxed, so health inspectors conduct most restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

- Total violations: 1
- Observed cheese sticks cold holding at improper temperature (62F). According to the manager, the trays were sitting at the room temperature to be cooked and then they were placed back in the cooler. Time/temperature control for safety (TCS) foods must be held at 41F or below to prevent growth of harmful bacteria.

- EHS observed ground sashimi grade raw tuna in reduced oxygen packaging (ROP) AKA vacuum packed, thawing without the vacuum seal broken. When thawing fish that is in ROP, the vacuum seal must be broken to introduce air before the thawing process begins so that growth of harmful anaerobic bacteria is eliminated. EHS demonstrated to person in charge (PIC) how to safely introduce air into packages.
- Three knives used on the line observed stored in between the two line coolers. In-use utensils, when stored in between uses, shall be stored on a clean dry surface. Store knives on a clean & dry portion of the line and not between equipment as those spaces are not easily and routinely cleaned.

- EHS observed one of the kitchen employees without a hair restraint. Food and food contacts must be protected from exposure to hair by wearing a hair restraint such as a hat or hair net.
- No date marking was observed within the facility. When keeping time/temperature control for safety foods (TCS) that are ready to eat (RTE) longer than 24 hours, a date that the food was prepared/opened or a discard date not to exceed 7 days shall be labeled on the food/container. Date label tamales, cut tomatoes, beef/chicken that has been cooked, cooked pastas and any other TCS/RTE foods.
- A container of raw beef was found stored directly on top of raw chicken breasts in the reach-in cooler. Food shall be protected from cross-contamination by separating raw animal proteins and arranging them in a way that cross-contamination risks are mitigated. Container of raw beef was moved from the top of the chicken and placed in a clean container before being stored above the raw chicken.

- High temp dish machine doesn't sanitize the kitchen utensils properly. The temperature of the utensils at the rack level is below 160 degrees Fahrenheit. The temperature is at 149 degrees.
- The cheese stick was not reheated to a sufficient temperature and time to eliminate pathogenic bacteria. The temperature of the cheese stick was observed at 98 degrees Fahrenheit.

- Different types of raw animal foods stored in such a manner that may cause cross contamination in that raw chicken observed stored above precooked vegetable samosa in the low boy refrigerator in the kitchen.
- Cut tomatoes registered at 58°F on food prep table in kitchen.
- The pipe under the three compartment sink observed leaking.

- There are no paper towels at the hand sink to dry hands. There must be paper towels available at hand sink for proper hand drying at all times.
- There is no available Chlorine sanitizer in dish machine. Sanitizer is reading 0 parts per million. Maintain 50-100 parts per million in a dish machine using Chlorine sanitizer for proper sanitization.
- Observed time/temperature control for safety foods (TCS) not properly dated for disposition. Foods not labeled include: Quiche, sausage. Ready to eat TCS foods held over 24 hours must be date marked with either made on date or date the food item must be consumed not to exceed 7 days.

- Total violations: 4
- EHS observed that hand sanitizer, not soap was the only cleaning provision provided at the hand sink in the kitchen. All hand sinks that are used by employees shall be provided with proper hand cleanser. Provide soap for washing hands.
- A container of Yogurt/dairy drink with an expiration date of 9/13 was found in the walk-in cooler. Milk products that are served must be of grade A standards. Milk products that have passed their expiration date that is printed on the container are no longer grade A. Person in charge voluntarily discarded the yogurt drink.
- A container of marinara sauce was found in the reach in cooler with a date mark of 9/01 with visible mold growing inside the container. Foods that are time/temperature control for safety (TCS) that are ready to eat (RTE) shall be held no more than 7 days from the day that that the food was prepared/opened, with that date counting as day one. PIC voluntarily discarded the marinara sauce.
- A container of HotShot Flying Insect aerosol pesticide was found stored within the shelving of the kitchen. Aerosol pesticides may not be used in a food establishment unless applied by a person whom is certified in that application. PIC voluntarily discarded the insecticide.

- Employee uncovered coffee cup was stored on the kitchen counter. Food employees may drink only from a covered container using a straw.
- Raw fish and chicken stored on top of the cut vegetables in the reach in cooler and on the kitchen counter. Prevent potential cross contamination by not storing raw animal foods over ready to eat foods.
- There is no available Chlorine sanitizer in dish machine. Sanitizer is reading 0 parts per million. Maintain 50-100 parts per million in a dish machine using Chlorine sanitizer for proper sanitization.
- Observed time/temperature control for safety foods (TCS) not properly dated for disposition. Foods not labeled include: cooked chicken, goat, lentil. Ready to eat TCS foods held over 24 hours must be date marked with either made on date or date the food item must be consumed not to exceed 7 days.
- A large container of tomato sauce observed cooling on the kitchen counter. The following methods used for cooling were not adequate to facilitate cooling of the product from 135°F to 41°F within 6 hours.
- The interior surfaces of the mechanical warewashing machine are soiled with (food debris, limescale) that may decrease the effectiveness of the unit.
- Facility doesn't have test strip to check the chlorine sanitizer at the dish machine.
- Multiple light bulbs on the kitchen ceiling observed without the shield.

- Two hand sinks blocked (by grill and by 3 compartment sink). Pans and brush stored in basins. Hand sinks shall be accessible at all times.
- Observed quat sanitizer at bar 3-compartment sink tested at 0ppm. Sanitizer shall be prepared at proper concentration and tested to verify safe.
- Observed mashed potatoes reheated for hot holding with an internal temperature of 153F. TCS (time temperature control for safety) foods shall be reheated to an internal temperature of 165F for at least 15 seconds.
- The following areas of the physical facilities in need of repair: Faucet base at hand sink in salad prep area is extremely deteriorated and edges are sharp and jagged, walk in cooler door does not shut completely and is observed with many holes in varying sizes on the exterior door, wall frame next to walk in cooler door is in disrepair. Physical facilities shall be maintained.

- A wet wiping cloth was observed stored directly on a prep table. Wet wiping cloths that are used to wipe down counters/equipment must be stored in between uses in a sanitizing solution. Person in charge had employee put the wet wiping cloth back into the sanitizing bucket.

- Employee observed sanitizing board and not wash her/his hand before engaging in food preparation.
- Ready to eat time/temperature control for safety (TCS) foods held over 24 hours are being kept past their consume by dates labeled on the containers. These foods are as follows: sliced tomato, multiple soups.
- Plastic rubber around the freezer door is coming off.
- Multiple ceiling light fixtures are not working and it's too dark in the kitchen.

- No paper towels provided at bar hand sink. Provide paper towels for drying hands after washing.
- Glasses washed in bar area dish machine are not sanitized. Dish machine is not properly dispensing sanitizer, sanitizer tested at 0ppm. Sanitizer container is full.
- Many gnats observed throughout bar area. The presence of insects shall be controlled to minimize their presence on the premises.
- Observed uncovered pans of beans, and many boxes of food items in dry storage room stored directly on floor. Many terrine pots stored on floor beneath shelving in dry storage room. Food and food contact items shall be stored in a protective manner, elevate off of floor at least 6".
- The following areas of the physical facilities in need of detail cleaning: Both restroom sink basins and the ceiling vents observed with debris build up. The physical facilities shall be maintained cleaned.

- Low temperature dish machine is not dispensing chlorine sanitizer. Food contact surfaces shall be sanitized before use, after cleaning. Person in charge changed the sanitizer and primed the machine. Now registers 50PPM chlorine.
- EHS observed a personal cell phone resting on a prep table as well as three separate employee lunch bags on a prep table. Food and Food contact surfaces shall be protected from sources of contamination by storing employees personal items away from food, and food prep areas. Items were moved to an employee area out of the kitchen.
- Several utensils were observed stored in a container with stagnant water. In-use utensils that are stored between uses must be stored on a clean dry surface, in water that is 135F or more, or in constantly running water with enough velocity to wash away food particles. Utensils and container were moved to the three basin sink to be rewashed.

- Observed milk 48F received an hour before EHS took temperature. The person in charge when asked stated there was not a delivery temperature taken at time of delivery. Milk should be receive at 45F or below at delivery.
- Observed a variety of foods in the walk in cooler that were uncovered. Foods should be covered if not in the cooling process to protect from environmental contamination.
- Observed improper cold holding temperature of arugula 48F in prep cooler 2 person in charge did not know how long the arugula was in prep cooler for.
- Observed bok choy 58F, raw shrimp 64F cooling in 4 door reach in cooler tightly covered. Foods cooling should be uncovered or loosely covered when in the cooling process.
- Observed raw fish and shrimp thawing at the 3 compartment sink not under running water. When thawing the food should be under running water.

- Follow up inspection conducted in response to cold holding violation cited at previous follow up and routine inspections. The toppings coolers is not able to maintain TCS (time temperature control for safety) foods at 41F or less. TCS foods cold holding at improper temperatures. Cut tomatoes 56F, sliced tomatoes 56F, cut leafy greens 57F The manager states it is because the restaurant is busy and the cooler lid is open for extended time periods throughout the day. EHS explained that even if the restaurant is busy the cooling equipment (refrigeration units) should be able to maintain TCS foods at 41F or less. The other prep cooler on the line is maintaining proper food temperatures.
